Background :.Hyperprolactinemia has been reported to be associated with abnormalities of carbohydrate metabolism.

The aim of this study is to evaluate the effect of hyperprolactinemia and cabergoline treatment on insulin sensitivity and changes in body mass index (BMI).

Study design :.38 obese hyperprolactinemic women were recruited and 30 healthy subjects were included as controls.

Patients were given cabergoline (0.5-1.5 mg) once weekly for 3 months.

Prior and after treatment, the following were performed, serum prolactin level.

calculation of BMI, fasting insulin, fasting plasma glucose and insulin sensitivity was assessed using homeostasis model assesment (HOMA).

Results :.Serum prolactin levels were significantly higher in obese hyperprolactinemic patients than in control group (890 ± 40mIU / L vs480 ± 50mIU / L, p < 0.001).

Fasting plasma insulin levels and insulin resistance index were higher in patients than in control group (23 ± 4/μU / L and 5.9 + 1.2 vs 21 ± 4μU / L and 5.4 + 0.5 respectively).

A positive correlation between hyperprolactinemia and insulin resistance index in all patients (r = 0.41, p value < 0.01).

Reduction of serum prolactin level after cabergoline therapy was associated with a significant decrease in the insulin resistance index (p value < 0.05) in all patients without a significant body mass index change.

Conclusion.

:The hyperprolactinemic state is associated with impaired insulin sensitivity which predispose to type 2 DM.

Correction of the hyperprolactinemia is associated with improved insulin sensitivity.
