This prospective study was designed to evaluate the impact of urgent cholecystectomy in the management of acute cholecystitis, and also to compare between urgent and elective cholecystectomy.

100 patients with acute cholecystitis underwent cholecystectomy during the period from January 1997 to June 2000.

Patients were divided into 2 groups, 50 patients each.

Group A included those patients treated with urgent cholecystectomy (UC) while group B treated by elective cholecystectomy (EC).

There was no significant difference between UC and EC in regards to the time of the operation and wound infection the mean time of hospital stays since initial diagnosis to surgery was 4- days in UC group compared to 34 days in EC group, with comparable postoperative hospital stays in the two groups.

In conclusion urgent cholecystectomy is safe and effective treatment modality for acute cholecystitis.

It dramatically reduced the patient's suffering and time of hospital stay.

Therefore, the adoption of 13Cfor non-complicated acute cholecystitis is to be recommended.
