يتم تقييم العلاج المناعي باستخدام الخلايا (T) المبلورة لمستقبلات هجينة مولدة للمضادات كعلاج محتمل لأورام الخلايا (B).
أظهرت التجارب السريرية الأخيرة نتائج واعدة.
و كلما زاد عدد المضادات المحتملة للترشح, أصبح اختيار المضاد المناسب أكثر تحديا لتصميم الدراسات و لتقييم أمثل لفعالية الخلايا (T) المبلورة للمستقبلات الهجينة.
المطلوب تقييم دقيق للمضادات للتأكد من أن الخلايا (T) المبلورة للمستقبلات الهجينة سوف تقضي على الخلايا الخبيثة بشكل تفضيلي مع ضمان أدنى مستوى سمية ضد الأنسجة الطبيعية.
تبلور الخلايا (B) سطحا محددا لمولدات المضادات التي يمكن أن تستعمل نظريا كأهداف لتصميم الخلايا (T) المبلور للمستقبلات الهجينة.
على الرغم من أن العديد م هذه المضادات يمكن أن تحفز استجابات خلوية مناعية فعالة داخل الجسم الحي, فإن تطبيقاتها السريرية ما تزال تمثل تحديا إذ أن المضادات الوحيدة التي تم اختبارها في التطبيقات السريرية هي (CD19) و (CD20).
نستعرض هنا مولدات المضادات للخلايا (B) لتصميم المستقبلات الهجينة كما نذكر العقبات التي يمكن أن تتخلل عملية التعرف على الأهداف الخلوية التي يمكن أن تكون مفيدة.
Abstract EN
Immunotherapy with T cells expressing chimeric antigen receptors (CAR) is being evaluated as a potential treatment for B-cell neoplasms.
In recent clinical trials it has shown promising results.
As the number of potential candidate antigens expands, the choice of suitable target antigens becomes more challenging to design studies and to assess optimal efficacy of CAR.
Careful evaluation of candidate target antigens is required to ensure that T cells expressing CAR will preferentially kill malignant cells with a minimal toxicity against normal tissues.
B cells express specific surface antigens that can theoretically act as targets for CAR design.
Although many of these antigens can stimulate effective cellular immune responses in vivo, their implementation in clinical settings remains a challenge.
Only targeted B-cell antigens CD19 and CD20 have been tested in clinical trials.
This article reviews exploitable B cell surface antigens for CAR design and examines obstacles that could interfere with the identification of potentially useful cellular targets.
