يعتبر ضيق فتحة الشرج اثر العمليات الجراحية في هذه المنطقة من المضاعفات الشهيرة بين المرضى و قد اشتمل هذا البحث على (32) مريض عانوا من ضيق فتحة الشرج اثر عمليات استئصال البواسير بطرق مختلفة .
هذا وقد عولجت حالات ضيق فتحة الشرج بعملية V-Y وقد وجد الاتي : كانت نسبة الشفاء بطريقة V-Y 90.6% اي 23 مريض (حيث تخلف 6 مرضى عن المراجعة ) اما باقي المرضى و عددهم ثلاثة فقد عانوا من مضاعفات هذه الطريقة تمثلت في تلف اطراف الجرح -عدم التئام الجرح .
وقد عانى هؤلاء المرضى من عودة ضيق فتحة الشرج مرة اخرى ، وقد تم علاجهم بطريقة تحفظيه .
مما سبق : نجد ان هذه العملية جيده ومفيده في علاج ضيق فتحة الشرج الناتج عن عمليات استئصال البواسير الشرجية ، و ان احسن طريقة لتحاشي ضيق فتحة الشرج بعد عملية البواسير هو اجرائها بطريقة صحيحة من البداية .
Abstract EN
Patients and method: this study was conducted on 32 patients admitted with post-haemorrhoidectomy anal stenosis.
All patients were subjected to operative treatment in the form of: anal dilatation, internal anal sphincterotomy at one or two sites, and Y-Vanoplasty under general or spinal anaesthesia.
Results: The postoperative hospital stay ranged from 3 to 7 days with average 5 days.
After Y-V anoplasty, three patients developed minor early postoperative complications (suture dehiscence in 1m>o patient and ischemic contracture of the edge of the flap developed in the second).
After 4 weeks patients showed a complete healing of the wound, and were satisfied with the results, and showed alleviation of their preoperative symptoms such as pain, constipation and bleeding.
P / R examination, showed that the anal canal of all patients was about to be normal After 6 months, all patients judged their clinical result as good and satisfactory.
Except two patients showed postoperative mild stenosis and constipation they slightly responded to mild laxative.
After one year, only these two patients out of 32 still complaining of mild stenosis and constipation and responded well to manual anal dilatation.
No incontinent cases recorded in our study.
