Abstract AR
لا يزال تأثير مؤشر كتلة الجسم (BMI) على نتيجة الحمل بعد حقن الحيوانات المنوية داخل الهيولى (الحقن المجهري) مثيرا للجدل.
و قد أجريت هذه الدراسة لتقييم تأثير مؤشر كتلة الجسم على نتيجة حدوث الحمل بعد الحقن المجهري و لفحص تأثير العوامل الأخرى على هذه النتيجة.
و قد أجريت هذه الدراسة بتحليل نتائج الحمل لعدد 349 امرأة خضعن للحقن المجهري وفقا لمؤشر كتلة الجسم : < 25 kg / m2 (وزن طبيعي)، 25-< 30 kg / m2 (زيادة الوزن) و ≥30 kg / m2 (السمنة).
و قد تم عمل جميع التحاليل الإحصائية من خلال تطبيق نماذج الانحدار اللوجستي و باستخدام معامل (OR).
و قد لوحظ وجود انخفاض كبير ذو دلالة إحصائية في معدل حدوث الحمل بين السيدات ذوات الوزن الزائد و السمنة حيث كان معامل (OR) 0.50 و 0.45 للنساء اللاتي يعانين من زيادة الوزن و السمنة على التوالي.
كما أظهرت نتائج الدراسة أن معامل (OR) كان أكثر انخفاضا بين النساء اللاتي كانت أعمارهن أقل من 27 عاما و أكثر من 33 عاما و النساء اللاتي يعانين من العقم الأولي، بينما لم يكن هناك أي تأثير لزيادة الوزن أو السمنة على نتيجة الحمل بين النساء اللاتي تم حقنهن بأكثر من أثنين من الأجنة.
ونستنتج من هذه الدراسة كما أشير إليه في دراسات أخرى وجود علاقة بين زيادة الوزن و السمنة و انخفاض حدوث الحمل بعد الحقن المجهري، كما تقترح هذه الدراسة بأن عمر المرأة و نوع العقم و عدد الأجنة المنقولة قد يؤثر في تعديل هذا الانخفاض الملحوظ في حدوث الحمل لدى هؤلاء السيدات.
Abstract EN
Background: The effect of body mass index (BMI) on pregnancy outcome after intra cytoplasmic sperm injection (ICSI) is still controversial.
Objectives: To evaluate the impact of BMI on pregnancy outcome after ICSI and to investigate the modification effect of other factors as age, hormonal profile ect.
Methodology : the study analyzed pregnancy outcome of 349 women underwent ICSI according to their BMI : < 25 kg / m2 (normal), 25- < 30 kg / m2 (overweight) and ≥ 30 kg / m2 (obese).
The associations were generated by applying logistic regression models.
Results : A significant reduction in positive pregnancy outcome was observed among overweight and obese women.
The adjusted odds ratio (OR) was 0.50 (95 %) confidence interval (CI) = 0.25-0.99), and 0.45 (95 % CI = 0.20-0.89) for overweight and obese women respectively.
These estimates showed more reduction among women aged < 27 years and ≥33 years and in primary infertile women.
The reducing effect of overweight and obesity on pregnancy outcome, however, was absent among women with more than two ET.
Conclusion : our study contributes to the reports linking overweight and obesity with decreased positive pregnancy outcome after ICSI, and provides suggestion that women’s age, infertility type and number of ET to modify this reducing effect.
