English Abstract

Pregnancy is associated with significant changes in thyroid function test because several physiologic complex changes take place during pregnancy which together tend to modify the global economy due to metabolic demands occur during pregnancy.

The recognition of abnormality in thyroid function test during pregnancy is important for the welfare of the mother as well as fetus.

The aim of this study was to assess the alternation in thyroid function tests and TPO Antibodies in each trimester in normal pregnant women and puerperal women as compared to non pregnant women in al Mafraq area.

Blood sample were drawn in AL Mafraq and Obstetrics and Gynecology hospital from 122 women in al Mafraq population; 86 normal pregnant that randomly selected, 16 in the early puerperium, and finally 20 healthy non pregnant control to compare the data Age range in all groups was (18-40).

The concentrations of thyroid stimulating hormone (TSH), Total truodo-thyronine (TT3), Total and free thyroxine (TT4 and FT4) and thyroid peroxidase antibodies (TPO Abs) of were measured by ELISA kits It was observed that mean TT4 increase significantly (P value < 0.05) in puerperal and pregnant women with mean values 12.66 ± 2.67 IU / ml and 11.95 ± 3.2 IU / ml respectively, where in control group it was 9.04 ± 1.45 IU / ml.

Also Mean TT3 values shows significant increase (P value < 0.05) in the pregnant and puerperal women relative to non pregnant women with mean values 1.98 ± 1.14 IU / ml, 1.8 ± 0.95 IU / ml, 1.14±0.32 IU / ml respectively, also TT3 levels shows significant increase (P value < 0.05) in the third trimester of pregnancy when compared to first trimester with mean value 2.3-4 ± 1.1,15 ± 0.8 IU / ml Free T4 mean values showed non significant decrease in pregnant and puerperal women relative to control group, with mean values 0.8 ± 0.4 ng / dl, 0.77 ± 0.18 IU / ml 0.9 ± 0.16 IU / ml respectively.

While TSH mean values were elevated in pregnant and puerperal women when compared with control group, with mean values 1.93 ± 1.6 IU / ml, 1.96±1.3 IU / ml, 1.46 ±0.84 IU / ml respectively.

Further more TPO levels were found to be increased significantly (P value < 0.05) in puerperal women according to pregnant with mean values 17.6 ± 8.7 IU/ml, 12.7 ± 8.6 IU/ml.

while in the control group was 13.2±7 IU / ml.

In addition it was found a positive correlation between TPO & TSH in all groups (r2 = 0.36).

When comparing pregnant for the first time and pregnant more than once it was observed that the difference in TT4, TT4, FT4, TSH and TPO levels were non significant.

In conclusion this study have shown that maternal thyroid function changed during pregnancy, the increase in TSH, TT4, TT3 and decrease in FT4 values correspond to previous reported studies The thyroid function tests in pregnancy should be interpreted against gestational age related reference interval to avoid miss- interpretation of thyroid function during pregnancy Also TPO-Ab status of pregnant women should be considered when constructing trimester-specific reference ranges because elevated TPO Ab levels are associated with higher TSH values.
