في هذه الدراسة نحاول التمييز بين مرض التهاب المفاصل الروماتويدي و التهاب المفاصل المصاحب للالتهاب الكبدي الفيروسي (سي) باستخدام أداة جديدة أكثر خصوصية لالتهاب المفاصل الروماتويدي مثل الأجسام المضادة للكيراتين.
و ذلك بانتقاء 50 مريضا إيجابيا لعامل الروماتويد و قد قسم المرضى إلى مجموعتين : المجموعة الأولى : 25 مريضا مصابون بمرض الالتهاب الكبدي (سي).
المجموعة الثانية : 25 مريضا يستوفون الخصائص التي وضعتها جمعية الزمالة الأمريكية للروماتيزم لتشخيص مرض التهاب المفاصل الروماتويدي.
و قد تم إجراء التحاليل الآتية : صورة دم كاملة–وظائف كبد–سرعة الترسيب–بروتين سي التفاعلي–الأجسام المضادة لفيروس (سي)–الأجسام المضادة للنواة–بالإضافة إلى الأجسام المضادة للكيراتين.
و قد تم انتقاء 15 شخصا أصحاء كمجموعة ضابطة لا يظهرون أي نتائج إيجابية لآي من التحاليل السابقة.
كما تم عمل أشعة عادية على اليدين لكل المرضى مع استبعاد من وجد به دلالة على تآكل العظام.
و عند الكشف سيرولوجيا عن وجود الأجسام المضادة للكيراتين وجد أنها تظهر بنسبة كبيرة في المجموعة الثانية (60 %) و بنسبة أقل في المجموعة الأولى (12 %) و لا تظهر في المجموعة الضابطة.
كما أظهرت النتائج أن خصوصية التشخيص بواسطة الأجسام المضادة للكيراتين كانت بنسبة 88 % بينما كانت حساسية التشخيص بنسبة 60 %.
و قد أظهرت هذه الدراسة أهمية و خصوصية الأجسام المضادة للكيراتين في تشخيص التهاب المفاصل الروماتويدي و كذلك في التفرقة بينه و بين التهاب المفاصل المصاحب للالتهاب الكبدي الفيروسي (سي).
Abstract EN
Hepatitis C virus (HCV) infection is often associated with extra hepatic manifestations among which arthropathy is common.HCV-related arthritis commonly present as rheumatoid-like arthritis, with positive rheumatoid factor (RF).
In this study, we try to distinguish between rheumatoid arthritis (RA) and polyarthritis associated with HCV infection using a marker more specific to RA than the rheumatoid factor (RF) namely ant keratin antibody (AKA) Serum AKAs were evaluated (by indirect immunofluorescence technique) in two groups of patients, all were RF seropositive.
Group I: 25 patients with HCV associated polyarthralgia or arthritis.
Group II : 25 patients with RA fulfilling the American college of Rheumatology (ACR) revised criteria.
In addition 15 healthy individuals served as controls.
All patients of group II as well as healthy controls were HCV seronegative.
Other investigations were done for all groups such as erythrocyte sedimentation rate (ESR), C reactive protein (CRP) and Antinuclear antibody (ANA).
Plain X ray of both hands were done for all patients to exclude the patients with bone erosion.
The study revealed that ant keratin antibodies were detected in 15/ 25(60%) patients with true RA and only 3 / 25(12%) patients with HCV-related arthritis.
AKA were not found in the sera of the healthy controls.
The specificity and sensitivity of AKA in RA group were 88% and 60% respectively.
Conclusion: Ant keratin antibody is highly specific for RA and its estimation may increase the diagnostic performance of RA.AKA is a useful marker in discrimination between patients with true RA and those with HCV-associated arthritis.
