التوحد هو اضطراب هام، عصبي نمائي في وظيفة الدماغ.
ضعف المعرفة بين الأطباء عن التوحد عند الأطفال يتعارض مع التمييز المبكر و التداخل اللازم و الذي من شأنه تحسين و تطوير التشخيص لاضطراب التوحد.
لتقدير مستوى المعرفة عن اضطراب التوحد بين مقدمي الرعاية الصحية العاملين في عينة من المؤسسات الصحية في مدينة بغداد، و لتحديد العوامل المؤثرة على هذه المعرفة تم تنفيذ دراسة مقطعية تحليلية في (18) من مراكز الرعاية الصحية الأولية، و اثنان من مستشفيات الأطفال التخصصية في مدينة بغداد خلال الفترة من السادس من شهر شباط و لغاية الحادي عشر من شهر أيار لسنة 2011.
تم اختيار هذه المؤسسات الصحية وفق تقنية بسيطة عشوائية لأخذ العينات من خلال جدول الأرقام العشوائية و تم جمع البيانات اللازمة بالمقابلة المباشرة مع الأطباء المشتركين و العاملين في المؤسسات الصحية المختارة، باستخدام استفتاء مركب لجمع المعلومات و التي تخص وجهين، الأول يخص المعلومات الديموغرافية الاجتماعية العامة للأطباء المشتركين، أما الوجه الثاني فيشمل أربعة محاور على شكل (20) سؤال لتكوين فكرة عن مدى معرفة الأطباء المدرجين عن التوحد عند الأطفال.
خلال فترة الدراسة (200) طبيب تمت مقابلتهم و سؤالهم و قد اشتملت على 110 (55 %) هم ممارسين عامين و 46 (23 %) منهم كانوا مقيمين أطفال و كذلك 24 (12 %) هم إحصائي طب الأطفال و 20 (10 %) منهم كان من أخصائي طب الأبوة و المجتمع.
و لقد أظهرت الدراسة أن معظم الأطباء المشتركين (% 95) قد سمعوا باضطراب التوحد، و لكن بمستويات مختلفة من المعرفة، مع أعلى معدل للنقاط للمحاور الأربعة سجل بين أخصائي طب الأطفال، بينما أقل معدل للنقاط لجميع المحاور وجد بين الأطباء الممارسين العامين.
حوالي (56,5 %) من المستجيبين وافقوا على أن أعراض و علامات التوحد تثبت في سن يتراوح بين (1-4) سنوات من عمر الطفل.
وجدت هذه الدراسة علاقة مباشرة بين مستوى المعرفة بالمحاور الأربعة و بين كل من الاختصاص و محل عمل الأطباء المشتركين.
عدة توصيات اقترحت لغرض تحسين مستوى المعرفة بين مقدمي الرعاية في المؤسسات الصحية عن التوحد عند الأطفال في العراق.
Abstract EN
Background : Autism is a serious neurodevelopmental disorder.
Poor knowledge about childhood autism among care providers could delay early recognition and interventions that could negatively affect prognosis.
Objectives : To assess the knowledge level of autism disorder among care providers working in a sample of health facilities in Baghdad and to determine the factors that may have influenced such knowledge.
Methods : A descriptive cross-sectional study with an analytic element was conducted in 18 primary health care centers and two specialized pediatric hospitals in Baghdad during the period from 6th February to 11th May 2011.
Simple random sampling was used to select the health care centers.
Data were collected by direct interview with the participant doctors working in the selected health facilities via a structured questionnaire for general socio-demographic information and autism domains to asses their knowledge level about childhood autism.
Results : 200 doctors were interviewed regarding their knowledge about childhood autism ; of those 110 (55 %) were general practitioners, 46(23 %) pediatric residents, 24(12 %) pediatric specialists, and a further 20 (10 %) were family and community medicine specialists.
Most participants (95 %) were aware of autism disorder.
More than half (56.5 %) of the respondents knew the correct age of establishment of autism, including signs and symptoms (1-4) years of age.
There was a highly significant statistical difference between specialty of participants and the mean scores of different autistic domains with the highest mean score recorded among the pediatric specialists and the lowest mean score among the general practitioners working in the primary health care centers.
Conclusion : The specialty as well as the working place of enrolled doctors highly influenced their knowledge level of autism.
