الهدف من البحث : يهدف البحث إلى تقييم معدل انتشار وهن العظام بين مرضى السكري من النوع الأول و دراسة بعض أوجه خصائص الفسيولوجية المرضية لوهن العظام السكري.
مادة البحث : لقد أجري البحث على خمسون مريضا بالسكري من النوع الأول و الخالي من المضاعفات و كان كل المرضى من الذكور و تتراوح أعمارهم من 18-33 عاما و كذلك اشتمل البحث على عشرين من الأصحاء كمجموعة ضابطة.
طرق البحث : قد تم قياس كثافة العظام على عظمة الكاحل بواسطة جهاز الموجات فوق الصوتية و تم قياس دلائل تكون العظام متمثلة في قياس مستوى إنزيم الفوسفاتيز القلوي و مستوى الاستيوكالسين بالدم و كذلك تم قياس نسبة الكالسيوم و الفوسفات و الهيموجلوبين السكري بالدم.
النتائج : - لقد وجد أن معدل انتشار وهن العظام بين المرضى المصابين بالسكري من النوع الأول كان بنسبة 62 % و كانت نسبة الإصابة بهشاشة العظام 8 %.
- لقد وجد أن مستوى الاستيوكالسين بالدم كان أقل في مرضى السكري مقارنة بالأصحاء.
- لقد وجد أن مستوى الاستيوكالسين كان أقل في مرضى السكري المصابين بوهن العظام عنه في مرضى السكري الغير مصابين بوهن العظام.
- و لقد وجد أن نسبة الهيموجلوبين السكري في مرضى السكري المصابين بوهن العظام كانت أعلى من مثيلتها في مرضى السكري الغير مصابين بوهن العظام.
الاستنتاج : و مما سبق نستنتج أن معدل انتشار وهن العظام بين مرضى السكري من نوع الأول كانت بنسبة 62 % و هشاشة العظام بنسبة 8 % و كذلك نستنتج أن وهن العظام السكري ناتج أساسا عن نقص العظام و كذلك عدم انضباط مستوى السكر بالدم.
Abstract EN
Aim of work : To evaluate the prevalence of osteopenia in patients with type 1 diabetes and its relationship to the metabolic control of diabetes.
Also, to study if there is any changes in the biochemical markers of bone formation in type 1 diabetic.
Materials and Methods : This study included 50 male patients with uncomplicated type 1 diabetes, their ages ranged from 18 to 33 years with mean + /-SD (25.3 + /-5.4 years) and also, sex and aged matched 20 healthy subjects were included as control group.
The bone mineral density was measured at the calcaneal bone for all participants using quantitative ultrasonometry.
Laboratory evaluation also was done, including glycosylated hemoglobin, serum levels of calcium, phosphate, alkaline phosphates and osteocalcin.
Results : The prevalence of osteopenia in patients with type 1 diabetes was 62 % and 8% of type 1 diabetics met the criteria of osteoporosis.
Type 1 diabetic patients had significantly lower levels of mean serum osteocalcin than the control group.
Also, type 1 diabetic patients with calcaneal osteopenia had significantly higher levels of mean serum glycosylated hemoglobin and significantly lower levels of mean serum osteocalcin than those diabetics without calcaneal osteopenia.
Conclusion : Osteopenia is frequent in patients with type 1 diabetes with prevalence of 62% and 8% of type 1 diabetics met criteria of osteoporosis.
This diabetic osteopenia results mainly from decreased bone formation and the poor metabolic control.
We recommend screening of type 1 diabetics for osteopenia and optimization of metabolic control to prevent osteoporosis.
