Abstract AR
الهدف : دراسة حالات إصابة أكياس مائية في الطحال عولجوا جراحيا بدون استئصال الطحال و بدون زرق أدوية قاتلة للطفيلي أثناء العملية الجراحية.
التصميم : دراسة سلسلة حالات.
المرضى و طريقة البحث : أجريت الدراسة على خمسة و عشرون حالة أكياس مائية في الطحال.
تمت إجراء عمليات جراحية باستئصال الكيس و بدون استئصال الطحال.
تمت مراقبة المرضى بعد العملية بصدد حدوث مضاعفات أو رجوع الأكياس.
النتائج : أظهرت النتائج أن عدم رفع الطحال في حالة إصابته بالأكياس المائية باتها عملية ليست صعبة و أسهل بكثير من استئصال الطحال و الذي بدوره يؤثر على جهاز المناعة و خاصة عند الأطفال مع مضاعفات أكثر بعد العملية من عدم رفع الطحال. ا?لاستنتاج : يظهر من هذه الدراسة البسيطة أن عدم استئصال الطحال ليس ضروريا و نكتفي برفع الكيس إذ أن استئصال الطحال ذو مضاعفات أكثر و خاصة عند الأطفال لتأثيره على جهاز المناعة و زيادة نسبة الالتهاب و الوفيات و بدون حقن الأكياس المائية بالمواد القاتلة للطفيلي لتجنب مضاعفات الأدوية القاتلة للطفيلي, و بطبيعة الحال بعد أخذ بنظر الاعتبار كافة الإجراءات الوقائية اللازمة أثناء التداخل الجراحي لمنع تسرب أو انتشار الأكياس المائية في جميع الحالات.
Abstract EN
Objective : the aim is to determine the outcome of spleen sparing procedure in treating splenic hydatid cystic disease without intracavitary injection of scolecidal agent.
Design : case series study.
Patients and method : 25 patients with splenic hydatid cystic disease, underwent splenic preservation operation without use of scolecidal agent during operation.
Results : follow up of all patients after 3 years showed that obliteration of the cystic cavity was complete and the spleen was retained successfully with smooth post-operative course.
Conclusion : preservation of spleen in splenic hydatid cystic disease is less invasive, safe, without problems of scolecidal agent injection and with good encouraging results.
