و ألاسترادايول إل الأهداف : استكشاف القيمة التشخيصية لمستوياتβ-hCG هرمون Progesteroneو ال CA-125 في التشخيص المبكر لحمل خارج الرحم.
و الطريقة : تم قيض مستويات β-hCG و هرمون ال Progesterone و ال GA-125ألايلايزا بواسطة تقنيه (لأربعين امرأة مصابه بالحمل خارج الرحم و 24امرأة سليمة بحمل طبيعي داخل الرحم و يعمر حمل (ELISA) الاستراديول يتراوح بين 4و 10 أسابيع، تمت الدراسة في مستشفى الكاظمية التعليمي بغداد- العراق خلال الفترة بين نوفمبر 2010 و يونيو عام 2011.
النتائج : أشارت الدراسة إلى وجود انخفاض معنوي ملحوظ في قيم الاسترادايول و ال β-hCGو هرمون ال Progesterone و الP≤0.05) CA-125 ) في أمصال الدم للنساء المصابات بالحمل خارج الرحم و كانت القيم كالتالي6.51±2.39U/mI 2.054±0.47ng/mI 72.57±12.27mIU/mI; 13.4±pg/mI على التوالي مقارنة بالنساء السليمات ذوات الحمل الطبيعي داخل الرحم و التي كانت قيم المتوسط الحسابي ± الخطأ المعياري للمتوسط الحسابي (Mean± SEM) كالتالي :74.25±18.5U/mI; 28.36±3.7ng/mI; 249.54±18.0mIU/mI; 112.7±23.6pg/mI على التوالي.
و عند استخدام ال CA-125 و بتركيز20.5 U/mI كمستوى فاصل (Cut-off value) لتشخيص حالات الحمل خارج الرحم، كانت ألانتقائية 75.7% و بحساسية مقدارها 100%و كانت القيمة التنبؤية الإيجابية 100 %و الطبية 71.7 % .
الخاتمه : أثبتت هذه الدراسة و باستخدام المنحنيات ال ROC، أن مستويا ت ال CA-125 و هرمون ال Progesterone (القياس لمرة واحدة) مفيدة في التشخيص المبكر لحالات الحمل خارج الرحم.
Abstract EN
Objective: The study was designed to investigate the predictive value of CA-125, progesterone, estradiol and β-hCG in the diagnosis of ectopic pregnancy (EP) and inevitable miscarriage.
Methods: Forty women with EP, 17 with intrauterine (IU) abortive gestation and 24 regular pregnant women (controls) were studied.
: Serum levels of CA-125, progesterone, estradiol and ß-HCG were measured by Enzyme Linked Immuno Sorbent Assay (ELISA) techniques in 40 symptomatic women with ectopic pregnancy, 17 with intrauterine (IU) abortive gestation and 24 regular pregnant women (controls) during gestational age between (4-12) weeks at Al-Kadhmiya Teaching Hospital, Baghdad, Iraq, between November 2010 and June 2011.
Results: Women with EP had significantly lower CA-125, progesterone, estradiol and ß-HCG concentrations, compared to both women with IU abortive pregnancy and controls.
When using CA-125 concentration of 25.5 U/ml as a threshold for the diagnosis of ectopic pregnancy, sensitivity was 75.7%, specificity 100%, the positive predictive value was 100% and the negative predictive value 71.4%.
When using a progesterone concentration of 5 ng/ml as a cut-point for the diagnosis of EP, sensitivity, specificity, positive and negative predictive values were 86.8%, 100%, 100%, 80%, respectively.
Conclusion: These results by using ROC curves.
The measurement of CA-125 and progesterone levels is useful in discriminating ectopic and intrauterine abortive from normal gestations.
