الهدف من هذه الدراسة هو تقدير معدل الأمراض و الأنماط الوراثية الخلوية للحالات الناتجة عن خلل في التركيبة العامة المتعلقة بالكر وموسومات و خصوصا في الأطفال المتواجدين بالمنطقة الغربية من المملكة العربية السعودية، بالإضافة إلى ذلك البحث في العلاقة بين العوامل البيئية، الولادية ، الطبية، السكانية و الاجتماعية.
إن معدل الإصابة بالأمراض المتعلقة بالخلل الكروموسومى لكل ألف طفل سعودي مولود يقدر ب 2,8 و 2,4 للمرض المنغولي.
كشف التحليل الوراثي الخلوي أن الخلل الشكلي نسبته (92 % )، و الذي هو أكثر من الخلل العددي الذي نسبته (8 %)، و من المعروف أن المرض المنغولي يعد الأكثر شيوعا، أما بالنسبة إلى الأمراض التي يكون فيها تكرر النسخة لثلاث مرات لكروم وسوم 13 و 18 و وحيد اكس، فتعد الأقل انتشارا، أما بالشبة للحالات النادرة التي كان فيها توافق بين الخلل الشكلي لكروم وسوم 7 و10 فهي زيادة غير معروفة، و على هذا فقد لوحظ أن هناك تركيبة عكسية لكروم وسوم 3.
سجلت حالات المرض المنغولي Trisomy 21 أعلى نسبة (94 % )، أما الأشكال الأخرى فتدل على أنه تم تأكيد أهمية العلاقة بين تقدم السن للأم، و الخلل الكروم وسومي.
في حين أن العوامل الأخرى لم يكن لها أي صلة مباشرة.
إن الحالات المرضية المتعلقة بالخلل الكروم وسومي صفة عامة، قليلة، و لكن حالات المرض المنغولي عالية بالمقارنة بالمستوى المحلي و العالمي.
Abstract EN
It is attempted to estimate the incidence of chromosomal disorders, constitutional karyotype and cytogenetic patterns of chromosomal abnormalities in the Western Saudi region.
Further, associations between socio-demographic, obstetrical, medical and environmental factors were also attempted.
Overall incidence of chromosomal disorders was estimated at 2.8 and 2.4 for Down syndrome per thousand Saudi live births.
Cytogenetic analysis revealed preponderance of numerical (92 %) aberrations than structural (8 %).
Down syndrome was the most frequent form of the chromosomal disorders ; in addition, trisomies of chromosome 13 and 18 and monosomy of X was found in lesser frequencies.
A rare form of chromosomal configuration with translocation of chromosome 7 and 10, extra marker chromosome and pericentric inversion in chromosome 3 were also observed.
Trisomy 21 was recorded to have high frequency (94 %) ; other forms like Robertsonian translocation and mosaicism were also detected in lesser frequency.
A significant association between advanced mother ages was confirmed, other factors have no bearing.
Findings are : The estimated incidence of overall chromosomal disorders is lower, but incidence of Down syndrome is higher compared to national and international frequencies.
Down syndrome is the most frequently encountered disorder and karyotypic techniques effectively help diagnosing in most of the cases.
