هدف البحث : أجريت هذه الدراسة المستقبلية الوصفية لتحديد معدل الوفيات الملاحظ عند الأمهات نتيجة للنزوف التوليدية في مشافي الخرطوم في الفترة بين 1 كانون الثاني 2007 و حتى 30 حزيران 2008.
طرق البحث : تم إجراء مراجعة فورية لحالات الوفيات عند الأمهات في جميع مستشفيات الولادة في الخرطوم.
تم تحديد سبب الوفاة بناء على المعطيات السريرية لكل حالة، تم إجراء تشريح الجثة في 3 حالات لأسباب طبية قانونية.
النتائج : تم تسجيل 130 حالة وفاة لدى الأمهات من أصل 105210 ولادة حية، حيث بلغ معدل الوفيات عند الأمهات 124 لكل 100000 ولادة حية، منها 107 حالات (بنسبة 82.3%) نتيجة لأسباب توليدية مباشرة، و 23 حالة (بنسبة 17.7%) نتيجة لأسباب غير مباشرة.
شكل النزف التوليدي السبب الرئيسي للوفاة في 29.2 % من الحالات، 76.3 % منها نتيجة لنزف بعد الولادة و الذي يحدث لدى المجموعات منخفضة الخطورة.
شكلت النساء عديدات الولادة 21 % من الوفيات فقط، فيما لم يلاحظ وجود أية قصة لشذوذات توليدية هامة لدى 68.8 % .
لم يمثل النزف سببا الوفاة عند 42.1 % من الوفيات، كما لم تخضع 39.5 % من الحالات لأجراء نقل دم.
حدثت معظم حالات الوفاة (68.4 %) خلال 24 ساعة الأولى من القبول بالمستشفى.
الاستنتاجات : تظهر هذه الدراسة أن الولادة في المنزل، التأخر في طلب العون الطبي، تدني مستوى الرعاية الصحية خلال المخاض، التأخر في اتخاذ التداخل المناسب، عدم توافر الدم ومشتقاته و ضعف نظام الإحالة الطبية هي عوامل الخطورة الأساسية الكامنة وراء الوفيات عند الأمهات.
Abstract EN
Objective: This is a prospective, descriptive, hospitalbased study, to determine maternal mortality ratio with regard to obstetric hemorrhage in Khartoum from first of January 2007 to 30th of June 2008.
Methods: Maternal deaths were immediately reviewed in all maternity units in Khartoum state hospitals.
Causes of death were diagnosed on clinical grounds; autopsy was done only for three medico-legal cases.
Results: There were 130 maternal deaths reported out of 105210 live births; maternal mortality ratio is 124/100000 live births, 107 cases (82.3%) were due to direct obstetric causes, while 23 cases (17.7%) were from indirect causes.
Obstetric hemorrhage was the main direct cause of death 29.2%, 76.3% due to post partum hemorrhage, it affects low risk groups, only 21% were grand-multiparae and 68.8% had no significant abnormal obstetric history, 42.1% did not present with hemorrhage, and 39.5% had no blood transfusion.
Most of patients (68.4%) died within first 24 hours from admission.
Conclusions: This study showed that home delivery, late consultation, suboptimal intra-partum or intraoperative care, late intervention, unavailability of blood or blood substances and poor referral system are the main factors behind maternal deaths.
