الهداف : تحديد المتغيرات المتوقعة التي تؤثر على الاستفادة من خدمات الرعاية الصحية الأولية في كل من القطاع العام و الخاص، و ذلك عن طريق دراسة، مجموعتين من العوامل : عوامل متعلقة بالأفراد و عوامل متعلقة, بمزودي الخدمة الصحية التي يعتقد أن لها تأثير على استخدام هذه الخدمات و الاستفادة منها.
الطريقة : استخدمت الدراسة الاستبانة لجمع البيانات من المراجعين الذين حضروا لكل من المراكز الصحية الأولية العامة و المستوصفات الخاصة في مدينة الرياض.
تم تصميم الاستبانة لجمع العديد من البيانات عن مجموعة من المتغيرات المتعلقة بالمستفيدين من هذه الخدمات، و كذلك معلومات من مزودي هذه الخدمات.
ثم استخدام كل من التحليل الإحصائي الوصفي، و كذلك تحمل الانحدار اللوجستي الثنائي لتحديد المتغيرات الهامة إحصائيا التي قد تؤثر في استخدام أو الاستفادة من هذه الخدمات في كلا القطاعين : العام و الخاص.
النتائج : أظهرت النتائج أن هناك مجموعة من العوامل المتعلقة بالأفراد " الدخل، و الحالة الصحية للمريض، و التعليم، و النوع، و مدى توافر تأمين تصحي، و جنسية المريض) و كذلك مجموعة أخرى من العوامل المتعلقة, بمزودي الخدمة الصحية (موقع المرفق الصحي، و فترة الانتظار، و مدى توافر خدمات صحية مساندة في المرفق الصحي، و ساعات العمل للمرفق الصحي، و وجود أطباء أخصائيين) لها تأثيرات هامة و ذات دلالة إحصائية توثر في استخدام كل من المرافق الصحية العامة و الخاصة و الاستفادة من خدماتها.
الخاتمة : تبرز الدراسة مجموعة من العوامل التي تدفع المرضى إلى استخدام المرافق الصحية الخاصة على الرغم من توافر مراكز الرعاية الصحية الأولية المجانية في القطاع الحكومي.
إن فهم مثل هذه العوامل من قبل واضعي السياسات الصحية في المملكة أمر بالغ الأهمية لتوفير الخدمات الصحية، و كذلك للتعرف على استخدام مراكز الرعاية الصحية الأولية في المملكة و مدى الاستفادة منها.
توصي الدراسة بالبحث في بعض المواضيع الهامة، و التي يلزم تضمينها في البحوث العلمية المستقبلية.
Abstract EN
Objective : To determine the variables which predict the utilisation of public and private primary health care services, incorporating a number of individual and provider-related variables which are thought to influence the utilisation of these services.
Methods : The study employed a self-administered questionnaire to collect data from patients attending both public PHC centres and private dispensaries (or outpatient clinics) in Riyadh city.
The questionnaire was designed to collect data on a number of variables related both to health services users and providers.
Descriptive statistics and binary logistic regression were used to determine the significant variables which may influence the utilisation of these services.
Results : The results show that a number of patient-related variables (income, health status, education, gender, the presence of health insurance and nationality) and provider-related variables (location of the health facility, waiting time, the availability of ancillary services, opening hours and the availability of specialised doctors) were statistically significant in identifying the variables which influence the utilisation of both public PHC centres and private outpatient clinics.
Conclusion : The study highlights the significant factors which prompt patients to utilise private outpatient clinics despite the availability of free public PHC centres in Saudi Arabia.
Understanding these factors by health policy makers is important in the provision and the utilisation of primary health care services in the Kingdom.
The study recommends significant areas for future research.
