Fever of unknown origin (FUO) with its long list of differential diagnosis including infections, neoplasms and collagen vascular diseases, remains a challenge to all clinicians.

We report a case of an infant who presented with fever of one month duration with physical finding of hepatomegaly only.

Extensive work up of laboratory and imaging investigations revealed a final diagnosis of liver abscess.

Despite the existence of many developed diagnostic tools for such cases, good clinical judgment can lead to the definitive diagnosis saving the patients and their physicians from this dilemma.

Although controversy exists on whether to treat liver abscess medically or by percutaneous or surgical drainage, our patient was treated with broad spectrum antibiotics for a total of 6 weeks duration and showed full resolution
