هدف البحث : تهدف هذه الدراسة إلى مقارنة فاعلية الكلوبيتازول بروبيونيت 0,05% كريم مع البيماكروليمص 1% كريم في علاج مرضى الحزاز المنبسط الفموي. طريقة البحث : أجريت الدراسة على ثلاثين مريضا يعانون من مرض الحزاز المنبسط الفموي من المرضى المترددين على العيادة الخارجية لكلية طب الأسنان – جامعة قناة السويس تراوحت أعمارهم بين 13 – 65 سنة و كذلك المرضى المترددين على العيادة الخارجية بقسم الجلدية و التناسلية بمستشفى جامعة قناة السويس و تم أخذ تاريخ طبي كامل و فحصهم إكلينيكيا و أخذ خزعة من كل مريض لتأكيد التشخيص ثم تم إعطائهم العلاج.
تم استبعاد المرضى اللذين يتعاطون الأدوية التي تسبب الآفات الفموية الشبيهة بالمرض أو لديهم الآفات الناتجة من حشوات أو تركيبات الأسنان.
بعد الفحص السريري الأولي تم تقسيم المرضى عشوائيا حسب نوع الدواء المعطى إلى مجموعتين : المجموعة الأولى : تكونت من خمسة عشر مريضا كل مريض أعطي بيماكروليمس 1% كريم مرتين يوميا لمدة أربعة أسابيع على المناطق المصابة من الفم.
المجموعة الثانية تكونت من خمسة عشر مريضا كل مريض أعطي كلوبيتازول بروبيونيت 0,05% كريم مرتين يوميا لمدة أربعة أسابيع على المناطق المصابة من الفم.
تم فحص و معاينة المرضى و تصوير الحالات قبل بدء العلاج ثم خلال الأسبوع الأول و الثاني ثم خلال شهر و شهرين و ثلاثة أشهر بعد نهاية العلاج.
في كل زيارة تم أخذ المؤشرات التالية : مؤشر شدة الآفة الفموية.
مؤشر شدة الألم.
مؤشر الإحساس بالحرقان.
و قد أدخلت البيانات التي أخذت من المرضى لعمل الدراسات الإحصائية في الحاسب الآلي لإظهار النتائج. النتائج : إن كلا من بيماكروليمص 1% كريم و كلوبيتازول بروبيونيت 0,05% كريم أظهرا فعالية واضحة في تخفيض كل المؤشرات السريرية و أيضا أظهرا نقصا واضحا في شدة و حجم الآفة الفموية المصاحبة للمرض. الاستنتاج : أن كلا من بيماكروليمص 1% كريم و كلوبيتازول بروبيونيت 0,05% كريم يمكن أن يستخدما بشكل فعال في السيطرة على الأعراض الناجمة عن الآفات الفموية المصاحبة لمرض الحزاز المنبسط الفموي إضافة إلى خفض شدة و حجم تلك الآفات الفموية.
Abstract EN
Objective : comparison between the effects of pimecrolimus 1 % cream and clobetasol propionate 0.05 % cream in the treatment of patients with oral lichen planus.
Materials and Methods : A randomized, controlled study was designed.
The study was divided into two phases : Phase 1 : (Treatment phase) consisted of topical treatment for one month.
Phase II : (Follow up phase) was of two month follow-up period intervals.
Thirty patients were included in this study divided into 2 groups Group 1 : Consisted of 15 patients in which the patients apply a thin layer (2 ml) of pimecrolimus 1 % cream, on the OLP lesion, twice daily for four weeks.
Group II : Consisted of 15 patients in which the patients apply a thin layer of the same amount of 0.05 % clobetasol propionate cream, on the OLP lesion, twice daily for four weeks.
OLP lesions of each patient were assessed clinically at baseline, 1 week,2 weeks,4 weeks, 1 month and finally at 2 months, for lesion severity scores, VAS scores and Burning sensation scores.
Results : The results of the present study revealed that both pimecrolimus 1 % cream and clobetasol propionate 0, 05 % cream can be used effectively in treatment of OLP.
Conclusion : The therapeutic effect of both drugs was comparable as no superior efficacy regarding pain control, lesion size reduction as well as disease severity was superior with one drug than the other.
