نقدم تجربتنا في ثمانية عشرة عاما (من 1987-2005م) في استخدام قثطرة بالبالون.
لإحداث ثقب في الحاجز بين الأذينين في انعكاس الشرايين العظمى للقلب.
تم علاج أربعة و أربعين مريضا بهذه التقنية كما تم تقسيمهم لمجموعتين (المجموعة أ-19 مريض) بالأشعة الصوتية للقلب، و (المجموعة ب-25 مريض) بالأشعة السينية.
بمقارنة الثقب بين الاذنين و تركيز الأكسجين في المجموعتين قبل و بعد القسطرة.
وجد أن الثقب زاد من (2.3± 0.6 ملم) إلى (4.9 ± 1.3 ملم) و زاد أيضا تركيز الأكسجين من (48 ± 8 %) إلى (72 ± 16.3 %) في كلا المجموعتين بزيادة معتبره إحصائيا.
و زاد كلا المتغيران استقلاليا بقيم احصائية متغيرة و لوحظ اضطراب في ضربات القلب في 3 مرضى بينما انفجر بالون القثطرة في حالة، ولكن دون مضاعفات و قد أصيب مريض بهبوط في التنفس (بسبب المورفين) احتاج لتنفس صناعي مؤقت.
توفيت حالتان إحداهما بسبب تجمع سوائل حول القلب، و الأخرى نتيجة ثقب الوريد الكبدي.
و توفيت حالتان بسبب حموضة مستمرة في الدم بعد نجاح القسطرة (بسبب تأخر الوصول للمستشفى).
و قد خلصنا إلى أن التقنية آمنة و فعالة و تنقذ الحياة حتى إجراء الجراجة.
و هي فعالة بالأشعة فوق الصوتية أو بالأشعة السينية.
Abstract EN
This study presents 18 years experience (1987 to 2005) of balloon atrial septostomy in patients with d- transposition of the great arteries.
Fortyfour patients underwent this procedure, under echocardiographic (Group A, n = 19) and fluoroscopic guidance (Group B, n = 25).
The atrial septal defect size and oxygen saturation, before and after the procedure were compared in all as well as in Groups A and B.
The atrial septal defect and oxygen saturation increased in all; from means f 2.3 ± 0.6 mm, 48 ± 8% to 4.9 ± 1.3 mm and 72 ± 16.3% after the procedure with statistical significance.
Both variables increased in the two groups independently with statistical significance.
Transient arrhythmias were encountered in 3 patients and balloon ruptured in one, without any sequel.
One had respiratory arrest after morphine dministration and required temporary intubation.
One patient died due to cardiac tamponade and another due to hepatic vein perforation.
Two late presenters expired due to persistent metabolic acidosis even after successful procedure.
Therefore, this concludes that balloon atrial septostomy is a safe, effective and life saving palliation, and serves as a bridge for surgical intervention.
It is equally effective under fluoroscopic and echocardiograph monitoring.
