المقدمة : أجريت هذه الدراسة التطلعية لتقييم مدى تأثير الكي الضوئي (بالديود- Diode Laser عبر الصلبة) على مرض الزرق الناتج عن نمو الأوعية الدموية على القزحية.
الطرق : تم انتقاء 20 عين لــ 19 مريض يعانون من الزرق الناتج عن نمو أوعية دموية على القزحية، و من ثم تمت معالجتهم بطريقة الكي الضوئي بجهاز الديود ليزر و ذلك من آذار 2002 إلى آذار 2003.
لقد عرفنا معدل النجاح في هذه الدراسة : بهبوط الضغط من 5-25 مم زئبقي في العين التي ترى أكثر من حركة اليد، أو التخلص من الألم في العيون التي ترى حركة اليد أو أقل، و كذلك الحاجة إلى عدد أقل من القطرات المستخدمة في تخفيض الضغط العيني لدى جميع المرضى.
و تتلخص طريقة عمل الكي بجهاز الديود باستعمال مسبار الجهاز فوق الصلبة 1.5 مم خلف حاشية القرنية الملونة، و يتم وضع 28 – 30 نبضة حول ما يقارب 270 درجة من محيط القرنية.
النتائج : تم متابعة 19 مريض لمدة عام من تاريخ المعالجة بالكي الضوئي، و كانت نسبة النجاح 74%.
و لقد تم إجراء 30 عملية كي بالليزر لــ 19 مريض (أي بمعدل 1.55) جلسة لكل مريض.
و بالنسبة للمضاعفات؛ فلم يحدث ضمور عيني لدى أي من المرضى و لم يحدث هبوط دائم في ضغط العين، أيضا لم يحدث التهاب عيني تعاطفي، غير أنه كان هنالك بعض الالتهابات العرضية أو نزف بسيط في الحجرة الأمامية في أول أسبوع و ذلك في 15 %.
الخاتمة : إن معالجة ارتفاع الضغط العيني بالكي الضوئي عبر الصلبة بجهاز الديود ليزر هي طريقة فعالة و آمنة لمرض الزرق الناتج عن نمو الأوعية الدموية على القزحية.
Abstract EN
Background: This prospective study was conducted to evaluate the effectiveness of contact trans-scleral diode laser cyclophotocoagulation (TSCPC) for Neovascular glaucoma.
Methods: 20 eyes of nineteen patients with Neovascular glaucoma on maximum medical treatment were consecutively treated by TSCPC.
Success rate was defined as IOP of 5-25 mmHg in eyes of VA> hand movement (HM); or relief of pain in eyes with VA= HM including blind eyes (NPL) and reduction of eye drops used in all eyes TSCPC treatment was performed with the center of the probe 1.5 mm behind the limbus, 28-30 pulses were applied for 270 degrees around the cornea.
Results: 19 patients were followed up for 1 year after initial treatment.
The overall success was about 74%.
Within one year 30 laser procedure was performed; mean 1.55 per patient.
As for complications, there was no phthisis bulbi, persistent hypotonia, or sympathetic ophthalmia; however hyphema and moderate inflammatory reaction each occurred in 15%.
Conclusion: Trans-scleral diode laser cyclo-photocoagulation is an effective method for the treatment of Neovascular glaucoma.
