الهدف : تهدف هذه الدراسة إلى التعرف على نمط الاعتلال العقدي اللمفي الخارجي و تحديد أنواعه النسيجية المرضية عند المرضى في سن الطفولة في منطقة عسير بالمملكة العربية السعودية.
المنهج : تم تحليل الخزعات المنجزة على الأطفال حتى سن الثانية عشرة في مستشفى عسير المركزي و تلك المحالة من المستشفيات المحيطة خلال 17 سنة في الفترة ما بين 1986-2002 م بطريقة استرجاعية، و لقد تم التعرف على توزيع الأنماط النسيجية المرضية المختلفة وفقا للعمر و الجنس و موضع العقدة اللمفية.
النتائج : بلغ مجموعة الخزعات التي تمت دراستها 148 خزعة، جاء جميع المرضى باعتلال عقدي لمفي خارجي كعلامة رئيسية و كانت منطقة العنق الأكثر شيوعا للإصابة (80.4 %) كما كان فرط النسيج اللمفي التفاعلي اللانوعي النمط الأكثر شيوعا (81 حالة 54.7 %) و تبعه التهاب العقدة اللمفية الحبيبومي (35 حالة 23.6%) فالورم اللمفي الخبيث (24 حالة، 16.3 %) فالنقائل (3 حالات، 3 %) و أخيرا الاعتلال اللمفي النوعي (5 حالات 3.4 %).
الاستنتاج : إن السبب الأكثر شيوعا لإحالة المرضى إلى عيادات دمويات و أورام الطفولة هو تضخم العقدة اللمفية.
معظم هذه التضخمات حميدة في طبيعتها و يشار إليها بالاعتلال العقدي اللمفي التفاعلي، أما الأخماج و الخباثة فحدوثها أقل أن توزيع الاعتلال العقدي اللمفي الخارجي بين الأطفال في منطقة عسير مشابه لما تم نشره محليا و دوليا.
Abstract EN
Objective : The purpose of this study was to identify the anatomic distribution and histological types of the peripheral lymphadenopathy in the paediatric age group in the Assir region of Saudi Arabia.
Method : 148 children, 12 years of age and below who presented with peripheral lymphadenopathy to the Assir Central hospital and had biopsies done over a 17-year period (1986-2002) formed the basis of this retrospective study.
Results : The commonest anatomical location of peripheral lymphadenopathy was the cervical region (80.4 %) The commonest histological diagnosis was non-specific reactive lymphoid hyperplasia 81 (54.7 %) followed by granulomatous lymphadenitis 35(23.6 %), malignant lymphoma 24 (16.2 %), metastasis 3 (2.0 %) and miscellaneous lymphadenopathies 5(3.4 %).
Conclusion : The majority of peripheral lymphadenopathy in childhood in the Assir region is due to reactive hyperplasia, a benign condition whilst other causes such as infections and malignancy occur to a lesser extent.
Cervical lymphadenopathy was the commonest observed in consonance with the pattern world wide.
