أجري هذا البحث لدراسة تقييم قوة عضلات الكتف الرافعة في مرضى الرثيان المفصلي باستخدام جهاز قياس ضغط الدم المعدل و كذلك لتقييم الاختلافات بين قراءات الباحثين و المرضى لبيان دقة هذه الطريقة و مدى إمكانية استخدامها أكثر من مرة لقياس قوة العضلات.
و قد أجري هذا البحث على 50 مريضا و مصابا بمرض الرثيان المفصلي.
حيث تم قياس قوة العضلات الرافعة لمفصل الكتف الأيمن و ذلك من الوضع جاليا.
و قد تم تعديل الجزء المطاطي الخاص بالجهاز حيث طوى على نفسه مرتين و تم الحفاظ على هذا الوضع باستخدام شريط ذو لاصق خاص.
و تم تنفيذ هذا الاختبار بوضع الجزء المطاطي على الجزء الأسفل من الذراع و تم رفع الذراع إلى درجة 45º و لكي يتأكد الباحث من هذه الدرجة كان يمسك بيده اليسرى منقلة خاصة بالمفاصل مضبوطة على 45º بينما يضع راحة يده اليمنى على الجزء المطاطي الموجود أسفل ذراع المريض و بينما يحاول المريض رفع ذراعه لأعلى يقاومه الفاحص بقوة مكافئة بحيث لا يرتفع ذراعه عن 45º.
و قد أظهرت نتائج هذا البحث عدم وجود أي فرق ذو دلالة إحصائية بين قياس قوة العضلات بواسطة 3 فاحصين و بعد قياس كل مريض 3 مرات مما يشير إلى أن هذه الطريقة تمتاز بأنها بسيطة، و سهل الحمل، و آمنة، و رخيصة، و يمكن استخدامها لأكثر من مرة.
كما أنه يمكن استخدامها لقياس قوة عضلات أخرى بالجسم، و بالإضافة إلى ذلك فهي تمثل طريقة عملية للتقييم الرقمي لقوة العضلات الرافعة بواسطة اختبار يتم عمله على سرير الكشف.
Abstract EN
Objective : To quantitatively assess the voluntary power of the shoulder abductor muscles in patients with rheumatoid arthritis (RA) in the clinical neuromuscular examination.
Also, to assess the inter-patient and inter-observer variation in evaluating muscle strength using a modified sphygmomanometer technique.
Methods : 50 patients diagnosed to have RA were subjected to this study.
All of them could hold their right arm in abduction against gravity, had no severe or painful synovitis or evidence of major destruction in that shoulder or elbow joints.
The cuff and bladder of a mercury sphygmomanometer, folded into 4 equal sections, were placed on the lateral lower third of the right arm.
The pressure inside the cuff was raised to a base line of 30 mm Hg, then the patient’s arm was abducted to 45° at the shoulder joint.
The patient was asked to hold that position and pressure was applied on the opposite side by the observer.
The test was performed by 3 observers and 3 measurements were taken for each patient.
Results : Estimates of inter-patient and inter-observer variations showed no statistically significant difference (p > 0.05) on comparing results using the ANOVA test.
Conclusion : The method proved simple, reproducible, portable, safe, unexpensive and can be applied to other muscles.
