أجري هذ البحث على 48 من المرضى الذين يعانون من أعراض القصور الوريدي المؤمن بالأطراف السفلية (50 طرف سفلي)، و قد أدخل هؤلاء المرضى وحدة جراحة الأوعية الدموية بالمستشفى الجامعي بأسيوط في الفترة من مايو 1995 و حتى فبراير 1996 و كان متوسط أعمار هؤلاء المرضى 40 سنة و نسبة الذكور إلى الإناث 3 : 1 كما تعرض الجانب الأيسر للمرض في 1 و 52 % من الأطراف و كان متوسط الشكوى من الأعراض 38 شهرا.
و قد تبين من الفحص الإكلينيكي لتلك الأطراف المريضة وجود استسقاء بالجلد في 78 %، و ألم في 70 %، تلون بالجلد في 60 %، و قرح في 50 %، ود و إلى في 46 % من الأطراف، كما تم فحص كل الأطراف بجهاز الدوبلكس الملون، كما خضعت جميعها لعمل أشعة ملونة بالصبغة على الأوردة العميقة بها و قد لعبت تلك الفحوصات دورا هاما في التعرف على وجود انسداد وريدي مزمن في تلك الأطراف و تحديد مستوى الانسداد بها، هذا بالإضافة إلى تقييم الحالة الوظيفية للأجواء المختلفة للجهاز الوريدي العميق.
و قد ثبت لنا من خلال تلك الدراسة مدى الدقة الفائقة التي يتمتع بها جهاز الدوبلكس في تشخيص مرضى القصور الوريدي المزمن بالطرف السفلي، كما اتضح لنا أيضا أهمية عمل أشعة ملونة بالصبغة على الأوردة العميقة للتأكد من حسن سريان الدم خلال تلك الأوردة و تحديد كفاءة الصمامات بها.
Abstract EN
Chronic venous insufficiency (CVI) of the lower extremities is a very difficult and challenging aspect of venous disease.
It commonly affects the working strata of the community with resultant low man power and national income reduction.
The new CEAP classification of CVI provides more accurate assessment of the underlying venous problem.
We reviewed 50 lower limbs of 48 patients with CVI.
Their mean age was 40 years.
Male incidence predominated (75 %).
The left side was affected in 52.1 %.
The mean duration of symptoms was 38 months.
Clinical examination of all limbs demonstrated edema in 78 %, pain in 70 %, pigmentation in 60 %, ulceration in 50 %, and varicose veins in 46 %.
Both duplex ultrasound and phlebography could define chronic obstruction and identify the function of individual vein segments.
In conclusion, duplex scanning is very helpful in diagnosis of patients with CVI.
Phlebography, despite being invasive, affords sure assessment of venous permeability and provides accurate grading of valvular incompetence.
