من المعروف أن 18 % من مرضى الذئبة الحمراء قد يصابون بنوبات صرعية.
كان الهدف من وراء إجراء هذا البحث هو الكشف عن مرضى الصرع الذين يصابون بشيبة الذئبة الحمراء و دراسة خصائصهم.
فعلى مدار 6 أشهر تمت متابعة 163 مريض بالصرع من خلال عيادة الأعصاب.
أصيب منهم فقط 7 بشبه الذئبة الحمراء و كانوا كلهن إناث تتراوح أعمارهن بين العقدين الثاني و الرابع.
فكان أكثر من 85 % من الحالات تعاني نوبات صرعية عامة مما له دلالة إحصائية هامة.
كانت فترة الإصابة بالصرع التي تزيد على 5 سنوات ذات مؤشر إحصائي هام في ظهور شبيه الذئبة الحمراء.
و كان تخطيط كهربائية الدماغ (رسم المخ) طبيعيا في 57 % من الحالات.
كما أوضحت الإختبارات المعملية إصابة كل مرض شبيه الذئبة الحمراء بالأجسام المضادة للحمض النووي، و الريبوكس النووي و الهيستون و الكارديوليبين و الفوسفوليين.
انتهت النتائج إلى أن العقاقير المضادة للصرع كالفينايتونين و الكارابامازبين و الفالبروات لها القدرة على إظهار الإستعداد لإصابة مرض الصرع بشيبة الذئبة الحمراء التي غالبا ما تستجيب للشفاء بالتوقف المؤقت عن تعاطي مضادات الصرع و إضافة 30 مجم يوميا من الكورتيزون عن طريق الفم، توصلت الدراسة في النهاية أن الإناث المصابات بالصرع و التي تتراوح أعمارهن بين عشرين و أربعين عاما و يعانون من نوبات صرعية عامة لفترة تزيد لديهن الاستعداد للإصابة بشيبة الذئبة الحمراء.
Abstract EN
It is quite known that 18 % of systemic lupus erythematosus (SLE) develop seizures.
The aim of this study was to detect the epileptic patients who developed drug-induced lupus (DIL) and to identify the criteria of such patients who are liable to develop SLE-like syndrome.
Over 6 months, 163 epileptic patients were followed up by the Neurology Clinic of Al-Azhar University Hospitals.
Among them only seven patients were found to develop DIL.
Those patients were all females ranging from the second to fourth decades of life.
More than 85 % of DIL cases had generalized seizures.
The latter finding was statistically significant in developing DIL.
The EEG pattern was normal in 57 % of cases who had positive antinuclear antibodies (ANA) antiriboxynuclease antibodies (ADA) antihistone antibodies (AHA) anticardiolipin (ACL) and antiphosphlipid (APL).
Most antiepileptic drugs e.g.
phenytoin, carbamazepine and valprote had the potentiality to induce SLE that improved greatly on drug withdrawal and adding prednisolone 40 mg daily.
This study concluded that female epileptic patients aged 20-40 years having generalized seizures of more 5 years duration are liable to develop DIL.
