This prospective, randomized, double blind study was designed to qualitatively evaluate the effect of transdermal nitroglycerine on the antinociceptive action of low dose epidural tramadol inpatients undergoing gynecological surgery.
Sixty women, ASA status I-1I, ' - operations with standard epidural anesthesia, were randomly allocated into four groups (n= 15 in each).
All patients were premedicated with 0.05 mg.kg'1 intravenous midazolam and received 20-ml ropivacaine 0.5% epidurally.
Thirty min after the epidural injection, either 5-mg transdermal nitroglycerine patch or transdermal placebo was applied.
The control group received epidural saline and transdermal placebo.
The tramadol group received epidural tramadol and transdermal placebo.
The nitroglycerin group received epidural saline and transdermal nitroglycerine patch.
Lastly, the tramadolr-nitroglycerin group received epidural tramadol and transdermal nitroglycerine patch.
Pain and adverse actions were assessed using visual analog scale.
Conclusively, although neither epidural tramadol alone (50 mg), nor transdermal nitroglycerin alone (5 mg.day-1) delayed the time of supplementary analgesic administration, the combination of both medications provided an average of ١2 hours of adequate postoperative pain relief following classical repair without significant perioperative adverse sequelae, supposing that the nitric oxide and µ-opioid receptors may potentiate their antinociceptive actions.
