The objective of this work was to study the clinical application of serum ferritin as a true independent marker of insulin resistance in patients with hyper-insulinemic states in type 2 diabetes and patients with polycystic ovary syndrome (PCOS).

80 female patients were enrolled in this study, 20 healthy controls (group J), 20 with PCOS (group 2), 20 with controlled diabetes (group 3), and 20 with uncontrolled diabetes (group 4).

All participant groups were matching each other as regard height and BMI kg/m2.

Yet, since patients with PCOS were during child bearing period, they had a significantly younger age.

Full clinical assessment, FBS, 2hpp, fasting and 2hpp serum insulin, HbAlc and serum ferritin level were done for everybody.

The usual causes for hyper ferritinemia were ruled out.

Fasting serum insulin was highly significantly elevated in both groups 2 and 4 (P < 0.001), whereas, only a significant higher value was present in group 3 (P < 0.05), when each compared with the control group.

However, regarding serum ferritin was highly significantly elevated in group 4 vs.

controls as well as group 3 vs.

4 (P < 0.001).

But, higher significant results were observed in group 2 and 3 vs.

controls and in group 2 vs.

4 (P <0.05), being the higher-most value in group 4.

Correlation study between serum ferritin and FBS, F insulin, HbAcl on the other hand: showed a highly significant relation in group 4 for all parameters, FBS in group 3, FBS & F insulin in group2, and a non significant result for controls.
