الهدف من الدراسة : هو مقارنة كفاءة استخدام عقار الروبيفاكين بتركيز 0.75 % و الهيالونيديز بتركيز 50 وحدة دولية بالملي (مجموعة أ) باستخدام عقار الروبيفاكين بنفس التركيز و عقار اللدوكين بتركيز 2 % (مجموعة ب) في التخدير الموضعي في عمليات المياه البيضاء.
طرق البحث : تم اختيار عدد 30 مريض من الدرجة الأولى إلى الثالثة طبقا للمواصفات التي وضعتها الجمعية الأمريكية للتخدير في الدراسة و قد تم تقسيمهم عشوائيا إلى مجموعتين متساويتين (15 مريض لكل مجموعة) و قد تمت مقارنة كفاءة التخدير الموضعي و ذلك بتقييم حركة العين و حركة الجفن بعد إعطاء التخدير الموضعي و درجة الحد من الإحساس بالألم و إمكانية إجراء الجراحة بعد إعطاء التخدير الموضعي و المضاعفات الناتجة عنه.
النتائج : كانت كفاءة التخدير الموضعي متكافئة في المجموعتين (مجموعة أ 86.7 % و مجموعة ب 73.3 %) بعد 10 دقائق من إعطاء التخدير الموضعي.
غير أنه من الملاحظ أن المجموعة أ كانت الأفضل من حيث عدم الاحتياج إلى إعطاء جرعة إضافية من التخدير الموضعي و لم تكن هناك مضاعفات تذكر في المجموعة أ و ب خلال أو بعد العمليات.
الخلاصة : استخدام عقار الروبيفاكين مع الهيالونيديز يعطي نفس الكفاءة في التخدير الموضعي مثل استخدام الروبيفاكين و اللدوكين معا و إن كان الأفضل من حيث عدم الاحتياج إلى جرعة إضافية.
Abstract EN
ropivacaine and 50 IU / ml hyaluronidase with 0.75% ropivacaine and 2% lidocaine during peribulbar anaesthesia for cataract surgery.
Design: Thirty study is a double blind randomized clinical trial.
Methods.
Thirty physieal status ASA*1-I1I patients were randomly allocated to two groups.
Ropivncaine/ hyaluronidase group received 5 ml of 0.75 % ropivacnine plus 50 IU ml-1 hyakirnoidasc (RH group) .
and ropivacaine/ lidocaine group received 5 ml equal parts of 0.75 % ropivacnine and 2 % lidocaine (RL group).
Speed of onset and quality of motor blockade were assessed using ocular and eyelid movement scores, and sensory blockade was assessed by the three- point scoring system.
Surgical and patient satisfaction, and incidence of complications were recorded Results : The groups showed no difference in the rate of onset or degree of akincsis achieved.
Akinesia scores Of < 4 by 10 minutes (min) were 86.7 % in RH group and 73.3 % in RL group.
The low rate of supplementary anaesthesia in RH group may make ropivacaine/ hyaluronida.se mixture superior in peribulbar block.
There were no differences between groups m patients satisfaction and in the incidence of minor complications.
Conclusion : ropivacainc 0.75 % with hyaluronidase 50.IU ml-1 give quality of peribulbar block equivalent to ropivacaine 0.75 % with lidocaine 2 %, However, It is superior as regards the need for supplenientary anaesihesia
