يعتبر مرض كينبوك أحد مسببات الألم و العجز في مفصل الرسغ و اختلفت الآراء حول مسبباته و الطرق المستخدمة في التشخيص و العلاج.
و كان هدف البحث تقييم دور الشق العظمي التقصيري لمعالجة المرض.
و تشكلت مادة البحث من 13 مريضا أجريت لهم هذه الجراحة و تم عمل تقييم إكلينيكي و إشعاعي قبل و بعد الجراحة مشتملا على 4 حالات أجري لها أشعة رنين مغناطيسي قبل و بعد العملية و اشتمل التقييم الاكلينيكي الألم وحدة الحركة و قوة القبضة و مقارنة التغيرات الإشعاعية قبل و بعد العملية و كانت نتيجة الجراحة ممتازة في 8 مرضى و جيدة في 4 مرضى و سيئة في مريض واحد و قد تحسن معدل الحركة و قوة القبضة و وجد أن التحسن الاكلينيكي يسبق التحسن الإشعاعي و كان من العوامل التي أثرت على نجاح الجراحة سن المريض و غياب الفصال العظمي الغضروفي و الجراحة المبكرة و أظهرت النتائج أن العملية مفيدة حيث أنها تعيد توزيع الأحمال على العظمة القمرية و تمت مناقشة النتائج مع آراء الباحثين الآخرين.
Abstract EN
Background : Kienbock’s disease (KD) is a painful disorder of the wrist with a multi factorial aetiology.
Controversy exists regarding aetiology, diagnostic modalities and treatment.
Objective : Evaluation of radial shortening osteotomy (RSO) for the treatment of Kicnbocki disease.
Patients and methods : 13 patients treated by RSO were clinically and radiologically evaluate pre and post- operatively including MRI 4 patients.
Results: The procedure was excellent in 8.
good in 4 and poor in one.
Pain and range of motion (ROM), improved.
Radiological improvement evidenced by Plain radiography and MR1 was satisfactory Clinical improvement precede radiological improvement.
The younger the patient, earlier surgery, absence of degenerative changes were in favor of good results.
Discussion : the results were discussed and compared with other reports on the subject.
Conclusion : RSO is a simple procedure which IS useful in treating KD by correcting ulna variance (U.V) and redistribution of load over the lunate bone.
