حدوث ذبذبة الأذين و خفقان الأذين أو زيادة ضربات القلب الفوق بطينية تعتبر أكثر مشاكل القلب حدوثا بعد جراحات الصدر الكبرى.
و العديد من الأسباب اقترحت و لكن دور كلا منها يظل غير واضح.
و هذه العوامل تتضمن نقص الأكسجين بالدم، زيادة الحامضية بالدم، نقص البوتاسيوم بالدم، انخفاض أو ارتفاع الضغط، نقص الهيموجلوبين بالدم، تأثير أدوية التخدير، إثارة العصب الحائر و إثارة الجهاز السمبثاوى.
الهدف من البحث : تقيم كفاءة و درجة الأمان في استخدام عقار الديلتيازيم عن طريق الفم في منع حدوث الإضطرابات الأذينية بعد جراحات الصدر الكبرى.
طرق البحث : شملت الدراسة 120 مريض خضعوا لجراحات صدر كبرى و تم توزيعهم عشوائيا على مجموعتين : المجموعة الأولى : تلقى جميع المرضى عقار الديلتازيم عن طريق الفم بجرعة 60 مج / 8 ساعات بداية من 24 ساعة قبل الجراحة و استمر لمدة أسبوع بعد الجراحة.
المجموعة الثانية : لم يتلقى المرضى أي عقاقير (مجموعة للمراجعة).
و تم تقييم مستمر لجميع المرضى لمدة 48 ساعة بعد العملية في العناية المركزة و لمدة أسبوع للقسم الداخلي.
و تم دراسة رسم القلب، غازات الدم، الضغط الشرياني، ضغط الوريد الرئيسي و تحاليل أملاح الدم و تم تعديل أي خلل في هذه التحاليل بصفة مستمرة.
النتائج : الإضرابات القلبية حدثت بصفة رئيسية في أول 48 ساعة بعد الجراحة و التي مثلت 86.2 % من إجمالي حدوث الاضطرابات.
التذبذب الأذيني كان أكثر الاضطرابات حدوثا بنسبة 46.66 % يتبعه زيادة ضربات القلب فوق البطينية بنسبة 30 % و الخفقان الأذيني بنسبة 10 %.
أكثر معدل لحدوث الاضطرابات حدث بعد الاستئصال الكامل للرئة بنسبة 42.1 % يتبعه حالات إزالة فص من الرئة بنسبة 28.57 % ثم عمليات الحيزوم بنسبة 21.5 % ثم التقشير للغشاء البلوري بنسبة 18.75 % بينما لم تحدث اضطرابات بعد إصلاح فتق الحجاب الحاجز أو إزالة أكياس رئوية.
معدل حدوث الاضطرابات الأذينية في مجموعة الديلتيازيم كان أقل بصورة ملحوظة عن مجموعة المراقبة (11.67 % مقابل 36.66 % بالتتابع).
الاستنتاج : عقار الديلتيازيم عن طريق الفم ذو كفاءة و على درجة عالية من الأمان كعلاج وقائي ضد اضطرابات ذبذبة الأذين في جراحات الصدر الكبرى.
Abstract EN
Background : The perioperative management of patients with cardiac arrhythmias and conduction disturbances is an important task in patients undergoing thoracic surgery.
Objective : The evaluation of efficacy and safety of oral Diltiazem in prevention of postoperative atrial arrhythmias after major thoracic operations.
Patients and methods : A prospective, controlled, randomized clinical study from April 2000 to May 2002, design was used to study 120 patients who underwent elective major thoracic operations.
The patients were randomly distributed between two groups, the first group received oral diltiazem (60 mg / 8 hrs) starting 24 hours preoperatively and continued for one week postoperatively, while the second group was taken as a control group.
Continuous postoperative monitoring in the first 48 hours was done in the 1CU for evaluation of ECG, P02, arterial blood pressure and CVP.
Laboratory assessment of blood gases, serum sodium and potassium was done each hour.
Continuous clinical assessment of the patients was done to avoid hypotension, hypovolemia, hypoxia and / or hypokalemia.
Results : Demographic data were comparable.
In both groups, arrhythmias occurred mainly in the first 48 hrs postoperatively, which account for (86.2 %) of total incidence of arrhythmias.
Atrial fibrillation was the commonest recorded arrhythmia (46.66 %), followed by supraventricular tachycardia (30 %) and atrial flutter (10 %).
The highest incidence of arrhythmias occurred after pneumonectomy (8 / 19, 42.) %), followed by lobectomy (14 / 49, 28.57 %), mediastinal operations (4 / 19, 21.05 %) and decortication (3 / 16, 18.75 %), while no arrhythmias were recorded after both bullectomy and diaphragmatic hernia.
The incidence of atrial arrhythmias in diltiazem group was significantly lower than in the control group (11.67 % vs.
36.66 % respectively, P value < 0.01).
No serious side effects were reported in the diltiazem group.
Conclusions : Oral diltiazem is effective and safe as a prophylactic treatment against postoperative atrial arrhythmias in major thoracic surgery.
