يعد الفيروس (هـ) كأحد أسباب التهاب الكبد الحاد و الذي ينتقل عن طريق الفم و يسبب مشاكل عديدة نتيجة لحدوث التهاب كبدي وبائي أو حالات متفرقة في التهاب الكبد الحاد و هو في ذلك يشبه الفيروس (أ) و لكن نسبة الخطورة تكون أكثر خاصة في السيدات الحوامل في الأشهر الأخيرة من الحمل.
لذا كان الهدف من هذا البحث هو دراسة دور هذا الفيروس في الحالات و مقارنته بالفيروسات الأخرى.
و قد تمت هذه الدراسة على 94 مريضا بالتهاب الكبد الحاد من المترددين على مستشفيات جامعة قناة السويس و الأزهر في الفترة من يناير حتى يونيو 2002 حيث أخذت عينات الدم و تم فصل المصل و إجراء اختبارات وظائف الكبد و تعيين الجسم المناعي المضاد (م) للفيروسات أ، ب، سي و هـ و أيضا الجسم المناعي المضاد (جي) للفيروس (هـ) فقط.
و قد كانت النتائج كالآتي وجد الفيروس (أ) في 38 حالة بنسبة (40.4 %) و الفيروس (ب) في 21 حالة بنسبة (22.3 %) و الفيروس (سي) في 3 حالات فقط بنسبة (3.2 %) أما الفيروس (هـ) وجد في 18 حالة بنسبة (19.1 %) و لم يتم تعيين أي دلالة للفيروسات في 19 بنسبة (20 %) و لذلك يلزم عمل بعض الفحوصات الأخرى لتعيين أسباب هذه الالتهابات مثل الفيروسات الأخرى (جي، أف) و غيرها.
أيضا يتم تعيين الجسم المناعي المضاد (جي) للفيروس (هـ) في 36 حالة بنسبة (38 %) و ذلك يدل على ظهور هذا الجسم المضاد مبكرا في حالة الالتهابات الحادة و أهمية ذلك في تشخيص هذه الحالات و قد أوضحت هذه النتائج إلى أن الفيروس (هـ) متوطن في مصر و هو أحد الأسباب الشائعة في حدوث التهاب الكبد الحاد و ينتقل عن طريق الفم.
Abstract EN
HEV is one of the causative agents of enterically transmitted non ABC hepatitis.
It is a worldwide public health problem and is involved in epidemic, sporadic and fulminant hepatitis similar to HAV infection in being acute, mild icteric and self limiting.
HEV has a high mortality rate (about 20 %) in pregnant women infected during the third trimester.
The aim of the present work was to study the role of different hepatitis viruses as a cause of acute hepatitis in Egypt.
Selected ninety four acute hepatitis patients with jaundice of less than 10 days duration and ALT level greater than 4 times the upper limit of normal were the subjects of this study.
Sera were tested for liver function, HAV-IgM, HBc-IgM, HCV-IgM, HEV-IgM and HEV-IgG.
Out of the 94 sera, 38 (40.4 %) were found positive for HAV- IgM, 21(22.3 %) For HBc-IgM, 3 (3.2 %) for HCV - IgM, and 18 (19.1 %) for HEV-lgM.
Mixed infection was found in 5 patients (2 cases HAV + HEV and 3 cases HBV+ HEV).
Nineteen cases (20 %) had non ABCE hepatitis.
Out of the tested 94 cases 36 cases (38 %) had HEV-IgG positive denoting that IgG antibodies of HEV appear early in infection and is used for the diagnosis of acute HEV infection.
This study concluded that HEV is one of the common causes of acute viral hepatitis in Egypt.
