المقدمة : إن التدخل الجراحي الأمامي طريقة جيدة و مباشرة لعلاج الكسور التهتكية للفقرات الصدرية القطنية. الغرض من البحث : تقييم طرق استخدام التدخل الجراحي الأمامي للكسور التهتكية للفقرات الصدرية القطنية و كذلك تقييم نتائج هذه الطرق و مضاعفاتها. الطرق و المرضى : اشتملت هذه الدارسة على 18 مريضا, تم إجراء جراحة لهم بواسطة استخدام المدخل الأمامي للفقرات الصدرية القطنية لعلاج الكسور التهتكية.
و قد اعتمد البحث على أخذ التاريخ المرضى و الفحص الإكلينيكي لكل مريض و استخدام الأشعة السينية و الأشعة المقطعية و الرنين و المغناطيسي لتقييم المرضى, و تم متابعتهم بعد العملية بالفحص الإكلينيكي و الأشعة لمدة تراوحت ما بين شهرين إلى 12 شهر لتقييم النتائج. النتائج : اشتملت هذه الدراسة على 12 ذكور و 6 إناث, تراوحت أعمارهم ما بين 21 إلى 54 عام بمتوسط حسابي 38.3 عاما.
و قد استخدم فى هذا البحث ثلاثة أساليب مختلفة من التدخل الجراحي الأمامي للفقرات الصدرية القطنية.
معدل المضاعفات في هذا البحث كان 16.7% أثناء العملية, 16.7% بعد إجراء الجراحة.
النتائج الإكلينيكية بعد الجراحة أوضحت وجود نتائج ممتازة في 7 مرضى, نتائج جيدة جدا في 3 مرضى, نتائج جيدة في مريض واحد, و نتائج ضعيفة في 5 مرضى. الخلاصة : تؤكد هذه الدراسة على ضرورة وضع الطرق الجراحية الأمامية للفقرات الصدرية القطنية على أنها من الطرق الفعالة التي يستطيع جراح المخ و الأعصاب استخدامها في علاج الكسور التهتكية للفقرات, و يعتمد نجاح هذه الجراحة على صحة اختيار المريض اللازم لهذه الجراحة و خبرة بالتشريح الجراحي و كيفية تجنب المضاعفات.
Abstract EN
The choice of optimal surgical procedure for thoracolumbar burst fractures remains controversial.
Objective: To evaluate the results of anterior surgical approaches to thoracolumbar burst fractures as regard ease of the technique, safety and incidence of complications, postoperative stability and functional outcomes.
Methods: A retrospective study of 18 pa ents with thoracolumbar burst fractures treated through an anterior surgical approach between March 2011 and 2012.
We used both the Thoracolumbar Injury Classification and Severity (TLICS) score and the Load Sharing Classification score for selection of cases.
Anterior corpectomy, grafting and Z-plate fixation were done.
Follow up of cases was documented as regard clinical and radiological results.
Results: There were 12 males and 6 females, with a mean age of 38.3 years.
Falls were the most common cause.
Transpleural thoracotomy approach done in 5 pa ents, thoracoabdominal approach in 9 pa ents and flank retroperitoneal approach in 4 pa ents.
2 pa ents died first day postopera vely due to DVT and pulmonary embolism.
Satisfactory results occurred in 10 pa ents, 5 pa ents had no clinical improvement and 1pa ent showed clinical deteriora on.
Conclusion: Anterior approach of the spine allows for one stage decompression, stabilization and deformity correction.
