لمقدمة و الهدف من البحث : يعتبر متوسط حجم الصفائح الدموية دلالة على نشاط الصفائح الدموية و العلاقة بين نشاط الصفائح الدموية و مستوى التحكم فى سكر الدم غير معروفة و لذلك يهدف هذا البحث لدراسة نشاط الصفائح الدموية و كذلك العلاقة بين هذا النشاط و مستوى التحكم فى مستوى السكر بالدم فى مرضى السكري من النوع الثاني. مادة البحث : اشتمل هذا البحث على 60 مريضا من النوع الثانى من المرض السكري تراوحت أعمارهم ما بين 40-65 سنة و كذلك اشتمل البحث على 20 شخصا من الأصحاء كمجموعة ضابطة تراوحت أعمارهم بين 35-65 سنة. طريقة البحث : لقد خضع جميع المرضى و الأصحاء للآتي : 1- تاريخ مرضي شامل. 2- فحص اكلينيكي كامل. 3- قياس سكر الدم صائم و بعد الأكل بساعتين. 4- نسبة الهيموجلوبين السكري بالدم.
5- صورة دم كاملة متضمنة عد الصفائح الدموية و نسبة الهيموجلوبين.
6- قياس متوسط حجم الصفائح الدموية كدلالة على نشاط الصفائح الدموية. و لقد تم تقسيم مرضى السكري إلى مجموعتين حسب نسبة الهيموجلوبين السكري كما يلى : المجموعة الأولى ذات الهيموجلوبين السكري 7% و كان عددهم 20 مريضا و المجموعة الثانية ذات الهيموجلوبين ≥ السكري 7% و كان عددهم 40 مريضا. نتائج البحث : 1- كانت هناك زيادة ذات دلالة إحصائية فى متوسط حجم الصفائح الدموية فى مرضى السكري مقارنة بالأصحاء.
2- كانت هناك زيادة ذات دلالة إحصائية فى متوسط حجم الصفائح الدموية في مجموعة المرضى ذات الهيموجلوبين السكري ≥ 7% مقارنة بالمجموعة ذات الهيموجلوبين السكري 7%. 3- كان هناك ارتباطا إيجابيا بين متوسط حجم الصفائح الدموية و كل من مستوى سكر الدم الصائم و بعد الأكل بساعتين و نسبة الهيموجلوبين السكري كل على حدى.
الاستنتاج و التوصيات : و مما سبق نستنتج أن هناك زيادة فى نشاط الصفائح الدموية فى مرضى السكري من النوع الثاني و تكون هذه الزيادة أكثر وضوحا فى المرضى الذين يكون السكر لديهم أقل انضباطا و لذلك فإن ضبط السكر بالدم قد يقلل من زيادة نشاط الصفائح الدموية و بالتالي قد يمنع أو يؤخر إمكانية حدوث مضاعفات الأوعية الدموية فى مرضى السكري.
Abstract EN
The mean platelet volume (MPV) is a marker of platelet activity.The increased platelet activity may play a role in the development of vascular complications of type 2 diabetes.
The association between platelet activityand glycemic control in diabetics is uncertain.
Therefore, We aim to assess MPV in patients with type 2 diabetes and to determine the correlation of MPV with the degree of glycemic control.
Patients and Methods: Platelet counts and MPV were measured in 60 Type 2 diabetic patients and 20 healthy subjects using an automated blood cell counter.
The fasting plasma glucose levels, two-hour postprandial plasma glucose levels and HbA1c levels were also measured.
Also, patients with type 2 diabetes were classified into two groups according to their HbA1c levels: group A with HA1c <7 % and group B with HA1c ≥7%.
Results: The MPV was significantly higher in diabetics compared to the healthy subjects (8.9 ± 1.1 fl versus 7.2 ± 0.9 fl (P< 0.001)).
MPV showed a strong positive correlation with fasting plasma glucose, two-hour postprandial plasma glucose and HbA1C levels (P<0.001 for each).The MPV in groupB (9.2±1.4 fl) was significantly higher than that of group A(8.4±1.3 fl;P = 0.003).Conclusion: We concluded that mean platelet volume is significantly higher in type 2 diabetes and this is more apparent in those with poorer glycemic control.Therefore, glycemic control may decrease the platelet hyperactivity and thus may prevent or delay possible diabetic vascular complications.
