للأديبوسيتوكينات العديد من الأهداف الأساسية الفسيولوجية فيما يتعلق بتوازن الطاقة و الجلوكوز و أداء القلب.
و يعتبرالأبلين و الفيسفاتين و الريزيستين و اللبتين من الأديبوسيتوكينات النشطة, و مع ذلك لم يتم توضيح العلاقة بينهما و بين مرض السكري النوع الثاني و مضاعفاته على القلب و الأوعية الدموية.
أهداف هذه الدراسة هي توضيح الأدوار المرضية للأديبوسيتوكينات على مرض السكري النوع الثاني و مضاعفاته على القلب و الأوعية الدموية و استجابتها بعد تطبيق محفز الإنكرتين. تم تسجيل 60 مريضا من مرضى السكري النوع الثاني و خمسة عشر شخصا سليم و غير مصاب بمرض السكري لإجراء البحث و قد أظهر البحث أن دلالات السمنة و ضغط الدم الانقباضي و الانبساطي و دلالات السكر في الدم قد زادت زيادة ذات دلالة إحصائية في جميع مرضى السكري نسبة إلى الأصحاء بالإضافة إلى اضطراب مستويات دهون الدم.
أيضا, لوحظ وجود زيادة كبيرة في مستوى الأديبوسيتوكينات في جميع مرضى السكري.
و قد أدى العلاج بمحفز الإنكرتين أو الميتفورمين إلى تحسن كبير في دلالات السكر في الدم, و في مستويات دهون الدم.
بينما أدى استخدام محفز الإنكرتين منفصلا أو مضافا إلى الميتفورمين إلى تحسن في مؤشر كتلة الجسم و ضغط الدم الانقباضى و الانبساطى و مستوى الأنسولين الصيامى, و الدهون عالية الكثافة و قياس مستويات الأديبوسيتوكينات.
و على الرغم من أن هذه النتائج تعكس دور الأديبوسيتوكينات في الفيزيولوجيا المرضية لمرض السكرى النوع الثانى و مضاعفاته على القلب و الأوعية الدموية و تثبت تحسن في مؤشر كتلة الجسم, و انضباط نسبة السكر في الدم, و قياسات ضغط الدم الشريانى بعد استخدام محفز الإنكرتين لكنه يلزم مزيد من المتابعة لتأكيد النتائج على عدد أكبر من المرضى.
Abstract EN
Adipocytokines achieve fundamental physiological targets with respect to energy balance, glucose homeostasis and cardiac performance.
Visfatin, resistin, apelin and leptin are active adipocytokines, however their relations to diabetes mellitus (DM) and cardiovascular functions are poorly elucidated.
The objectives of this study are to clarify the relation of adipocytokines in type 2 DM and cardiovascular risks (hypertension &dyslipidaemia) and their responses after application of incretin enhancer.
Methods: Type 2 DM pa ents and fi een healthy controls were enrolled.
Pa ents were randomized to receive the usual regimen (me ormin 850 mg three mes /d orally) in group I (n=19), incretin (Vildaglip n 50 mg twice/d) in group II (n=17) or the combination of both agents in group III (n= 17) for 14 weeks.
Results: The adiposity related parameters, SBP, DBP, glycaemic control parameters increased significantly (p<0.001) in all diabe c groups relative to the controls in addition to disturbance of lipid profile.
Also, a significant increase in the level of vasfatin, resistin, leptin and apelin in all diabetic patients was observed.
Both incretin enhancer and metformin led to significant improvement in glycaemic parameters (fasting blood glucose and glycosylated haemoglobin), lipid profile (total cholesterol and triglycerides) with more noticeable effects after incretin application.
The addition of incretin enhancer seemed to confer greater benefits in amelioration of Body mass index (BMI), SBP, DBP, fasting insulin level, HDL and the measured adipocytokines levels in group II and III.
However, the advancement get more in the combination group which; suggest the complementary functions of both agents.
Aforementioned changes in adipocytokines of controls were correlated positively with BMI, SBP, DBP, FBG and HBA1c but they non-significantly related to other anthropometrics and laboratory parameters.
At baseline, significant positive correlations between adipocytokines with BMI, FBG and HBA1c in T2 DM pa ents were evident.
These results reflect involvement of adipocytokines in pathophysiology of T2 DM and the associated cardiovascular risks and improvement was proved in BMI, glycaemic control, and cardiovascular complications after application of incretin enhancer nevertheless, the overall clinical monitoring of incretin enhancer must be further assessed in wider scale.
