الهدف من البحث : دراسة مدى انتشار التوقف الانسدادي للتنفس أثناء النوم في مرضى السكري من النوع الثاني و تأثيره على مستوى ضبط السكر بالدم. مادة و طرق البحث : لقد أجري هذا البحث على 52 مريضا من مرضى السكري من النوع الثاني المترددين على عيادة السكر بمستشفى أسيوط الجامعى و لقد خضع جميع المرضى للآتي : 1.
تاريخ مرضى شامل. 2.
كشف إكلينيكى كامل. 3.
تسجيل التغيرات التي تحدث في عملية التنفس أثناء النوم بمعمل دراسات النوم بمستشفى أسيوط الجامعي. 4.
قياس الهيموجلوبين السكري بالدم. نتائج البحث : لقد أظهرت الدراسة أن معدل انتشار التوقف الانسدادى للتنفس أثناء النوم بين مرضى السكري من النوع الثاني كانت 62% و أنه كانت هناك علاقة إيجابية ذات دلالة بين معدل حدوث التوقف الانسدادى للتنفس أثناء النوم و الهيموجلوبين السكري و كذلك كانت هناك علاقة إيجابية ذات دلالة بين درجة خطورة التوقف الانسدادى للتنفس أثناء النوم و مستوى الهيموجلوبين السكرى بالدم. الاستنتاج و التوصيات : و مما سبق نستنتج أن التوقف الانسدادى للتنفس أثناء النوم يكثر بين مرضى السكري من النوع الثاني و أنه كلما زادت درجة خطورته ساءت حالة ضبط السكر بالدم ممثلة فى زيادة نسبة الهيموجلوبين السكري بالدم و لذلك نوصي بالكشف عن مرض التوقف الانسدادى للتنفس أثناء النوم في مرضى السكري من النوع الثاني وعلاجه مما قد يكون له أبلغ الأثر في التحكم فى مستوى السكر بالدم.
Abstract EN
Obstructive sleep apnea (OSA) is a highly prevalent comorbidity of type 2 diabetes.
The aim of this work is to determine the risk of OSA in patients with type 2 diabetes and its effect on the glycemic control.
Patients andMethods: This study was conducted at Assiut UniversityHospital Sleep Unit, 52 patients with type 2 diabetes were included, all patients underwent an overnight polysomnography (PSG) and glycosylated hemoglobin(HbA1c)measurement.
Results: A total of 62% (n=32) of patients with type 2 diabetes had OSA(apnea-hyponea index (AHI) ≥ 5 events/hour).
27% (n=14) of patients had mild OSA(5≤AHI <15) , 19% (n=10) had moderate OSA (15 ≤ AHI <30) and 16% (n=8) had severe OSA (AHI ≥30).
There was a significant positive correlation of HbA1c levels with apnea hypopnea index, a widely accepted marker of the severity of OSA (P<0.001).
Also, we found that increasing OSAseverity was correlated with increasing HbA1c levels (p<0.001).
The mean HbA1c values were 8.2% for those without OSA, 9.4% for mild OSA, 10.1% for moderate OSA and 10.6% for severe OSA.
Conclusion: We concluded that the prevalence of OSA in this study was 62% and increasing severity of OSA is associated with poorer glycemic control.
Therefore, identifying and treating OSAin patients with type 2 diabetes may improve their glycemic control.-
