تعتبر الكيسات العدارية حالة متوطنة في دول البحر الأبيض المتوسط بما فيها سوريا.
و بينما يصاب الجهاز العصبي المركزي في 2-3 % من الحالات، تبقى اصابة المخيخ بالكيسات العدارية نادرة جدا.
نعرض في هذا التقرير حالة تظاهرت بشكل إسعافي بأعراض ارتفاع التوتر داخل القحف مع تدن في مستوى الوعي على سلم Glascow للسبات.
أدت المراقبة الحثيثة للحالة و المقاربة الجراحية الدقيقة بحسب تقنية Dowling’s إلى نتائج ممتازة و شفاء كامل.
نكد على أهمية العلاج الجراحي الحذر لهذه الحالات و ذلك لأن تمزق الكيسة و تسرب محتوياتها يسبب داء الكيسات العدارية الثانوي الأمر الذي يسيء بشكل كبير للإنذار.
يجب أن توضع الكيسات العدارية بالحسبان عند وضع التشخيص خاصة في البلدان التي يتوطن فيها هذا الداء.
Abstract EN
The hydatid cyst (HC) is an endemic in Mediterranean region including Syria.
The central nervous system is involved in 2-3 % of cases.
However, hydatid cyst in cerebellum is very rare.
We report a case that presented as an emergency for high intracranial pressure and deteriorating Glascow Coma Scale.
Close monitoring and precise surgical management using Dowling’s technique resulted in very good outcome with full recovery.
We highlight the need for very careful surgical treatment because cyst rupture and secondary hydatidosis due to spillage of the cyst contents can dramatically worsen the outcome.
Hydatid cyst should be taken into consideration specially in countries where hydatid infestation is endemic.
