خلال (16) سنة تمت معالجة (60) حالة من إصابات مفصل الركبة بالمقنوفات النارية ذات السرع الواطئة و الباقية في المفصل بواسطة العلاج الجراحي المبكر و الذي يشتمل على تنظيف الجروح و رفع المقذوف الناري من داخل الركبة و تثبيت القطع العظمية الغضروفية كبيرة الحجم و المنفصلة من جراء الإصابة بالمقذوف أن من بين الأهداف لإجراء مثل هذا العلاج هو منع حدوث المضاعفات التي قد تحصل نتيجة بقاء المقذوف الناري داخل المفصل أو نتيجة وجود القطع العظمية الغضروفية المنفصلة داخل الركبة فضلا عن مكون هذا البحث يهدف إلى تصميم بروتوكول خاص بعلاج مثل هذه الحالات.
أن الإصابات شملت 54 من الذكور 6 من الإناث كما أن الأعمار تراوحت بين 6 الإناث كما أن الأعمار للمصابين تراوحت بين 6 إلى 36 سنة.
أن سبب الإصابة كان شظايا نتيجة انفجارات لقنابل في 40 (66.7 %) حالة و طلق ناري في 20 (33.3 %) حالة كما أن مكان المقذوفات داخل الركبة كان في غالبية الحالات في الجزء الأمامي للركبة و هذا الجزء كان هو الذي يتم فتحه و استكشافه أولا في جميع الحالات و ذلك من اجل رفع المقذوفات أو القطع العظمية الغضروفية حتى في حالات وجود المقذوف في الجزء الخلفي للركبة لتفادي مضاعفات إحداث جرحين استكشافيين أمامي و خلفي.
أن المتابعة الطويلة للحالات التي أجري لها العلاج الجراحي المبكر ضمن بروتوكول مصمم لهذا الغرض أثبتت جدارة و نجاحا من اجل تقليل المضاعفات المحتملة لمثل هذه الإصابات فضلا عن حصول المرضى على نتائج وظيفية جيدة.
Abstract EN
During the last sixteen years, sixty patient with single retained low velocity missile injuries of the knee joint were treated by early surgical management which included debridement , missile extraction , fixation of large detached osteo-chondral fragment and removal of small piece The objectives were to avoid the possible complications which may result from long intra-articular stay and those due to presence of dislodged osteochondral piece and in addition ,this study aimed at providing a protocol for the management of such injuries .54 ( 90%) injured patients were males and 6, (10% ) females .
Age ranged between 6-36 years .
The cause of injuries was shell fragment due to bomb explosion in 40 ( 66.7% ) patient and bullets in 20 ( 33.3% ) .
The most common missile location was in the anterior compartment which was first explored in i all patients for missile removal and management of dislodged osteochondral fragment.
long time follow up in this study proved that early surgical management as part of a designed protocol minimizes the possible complications of such injuries and results in good functional recovery of the injured patients .
