لم يعد سرطان الثدي مرضا واحدا بل له عدة أوجه و كل منها يتكون من أنواع مختلفة.
و يختلف كل نوع من هذا المرض باختلاف الطبيعة و التاريخ المرضي و العديد من العوامل الإكلينكية الباثولوجية و المستقبلية للمرض.
اتجه الاهتمام موخرا إلى تقسيم مرض سرطان الثدي بحسب التكوين الجزيئ للورم. هدف البحث : - المقارنة بين أربعة أنواع من سرطان الثدي من حيث التغيرات الإكلينكية و الباثولوجية. - تم تقسيم حوالي 255 حالة مرض سرطان الثدي (المرحلتين الثانية و الثالثة) إلى أربع مجموعات باستعمال صبغة الأميونو الخاصة بحسب وجود مستقبلات الهرنومات الأتية الأستروجين و البروجستيرون و عامل هير -2. النتائج : - تكون المجموعة التي تفتقر إلى مستقبلات الهرمونات و عامل هير -2 الغالبية الكبرى من الحالات 76.5% و تتميز بارتجاع المرض المتكرر. - ما زال الاكتشاف المبكر لسرطان الثدي هو العامل المؤثر في الشفاء في جميع الحالات.
Abstract EN
Introduction : Breast cancer is no longer seen as a single disease but rather a multifaceted disease composed of distinct biological subtypes with diverse natural history, clinical, pathological and molecular features.
Recent attention has been directed at molecular classification of breast cancer.
Objective : To evaluate the prognostic value of triple negative subtype in stage 11 / 111 breast cancer.
To define the role of clinical stage in prognosis of breast cancer Methods : We used immuno- histochemical technique to divide 255 cases of breast cancer, stages II and III into four subtypes according to estrogen receptor / progesterone receptor (ER / PRj and Iler-2 expression.
Results : Triple-negative subtype comprised 76.5 % of the cases with 12.3 % recurrence rate.
Luminal A subtype also carried a poor out come with 16.7 % recurrence rate.
Conclusion : Triple negative subtype has the worst overall and disease-free survival in stage ITIII breast cancer.
Clinical stage is still an independent prognostic factor in breast cancer of all types.
