يتسبب تثبيت الفقرات كعلاج جراحي لبعض مرضى عدم اتزان الفقرات أو انحراف العمود الفقري في نقص المجال الحركي لوحدة الحركة الفقارية.
يفترض البحث أن هذا قد يؤدى إلى نقص في الخرج الحسي العميق من العمود الفقري و الضروري لأنماط سليمة في السيطرة على القوام.
هدف البحث : استقصاء محددات استجابة القوام الآلية في الساق و الجزع في مرضى أجريت لهم عمليات تثبيت للفقرات.
طريقة البحث : أجرى البحث على 16 مريض انزلاق فقاري قطني و 15 مريض جنف أجريت لهم جراحة لتثبيت الفقرات.
قورنت استجابات القوام الآلية لهم عقب أحداث اضطراب قوام للخلف مع استجابات 20 متطوعا من الأصحاء.
النتائج : تبين وجود شذوذ ذو دلالة إحصائية لاستجابات القوام الآلية في مجموعة المرضى مقارنة بالأصحاء.
تمثل الشذوذ في تأخير زمن الكمون للعضلات الباسطة للجزع و كذلك ازدياد الفجوة الزمنية بين استجابة الساق و استجابة الجزع.
إضافة، كان هناك نقصا في قيمة الذروة للعضلات الباسطة للجزع في مجموعة المرضى.
أخيرا، كان هناك دليلا إحصائيا لارتباط متين بين استجابة عضلات الجزع و الساق مقترحا نمط تيبسي لم تتم ملاحظته في المجموعة الضابطة.
الخلاصة : تقترح النتائج وجود مشكلة في المعلومات الحسية العميقة الآتية من الفقرات المثبتة و التي من المحتمل إن تكون مسئولة عن شذوذ الاستجابة الملاحظ في هذه الدراسة.
قد يؤدى هذا الشذوذ إلى حمل ميكانيكي غير مسيطر عليه على اتزان كل من الجزع و الجسم ككل.
Abstract EN
Background : Spinal fixation is a commonly used-procedure to manage spinal instability and mal-alignment as in spondylolisthesis and scoliosis.
Restriction of the movement of the spinal segment is among its mechanical consequences.
This restriction is assumed to reduce the proprioceptive output from the spine that is essential for proper postural control patterns.
The aim of the work : was to investigate the parameters of the automatic postural response of the leg and trunk in patients who have undergone spinal fixation.
Materials and Methods : Sixteen patients whose spine was fixed for management of spondylolisthesis and another 15 patients who had spinal fixation for idiopathic scoliosis were studied.
Their automatic postural response parameters following backward perturbation of their erect stance, were compared with the response from 20 healthy volunteers.
Results : The study showed evidence supportive of significant deviation from normality of the automatic postural response of the patients’ group.
The abnormality was in the form of delayed erector spinae response latency as well as increased time gap between the trunk response and the leg response.
Additionally, there was erector spinae response amplitude reduction in the patients ‘group.
Finally, there was a statistical evidence of a tight link of the leg and trunk muscle response suggesting a stiffening pattern that was not observed among the control group.
Conclusion : The results suggest a problem with the proprioceptive information coming out of the fixed spine that may probably be responsible for the response abnormality observed in the study.
These abnormalities may put uncontrolled mechanical burden both on the trunk and the overall body balance.
