إن الإصابة بالحمى الروماتيزمية تتبع الإصابة بالبكتيريا العنقودية و يعتقد أيضا أن هناك صلة بين احتمال الإصابة بالحمى الروماتيزمية و بين تناسق أنسجة خلايا الدم البيضاء.
الغرض من هذا البحث محاولة اكتشاف العلاقة بين الإصابة بالحمى الروماتيزمية و جين تناسق أنسجة خلايا الدم البيضاء المجموعة I.
و قد أجريت الدراسة على 20 طفل يعانون من الحمى الروماتيزمية بالمقارنة ب 20 طفل أصحاء و تم فحصهم إكلينيكيا و عمل تحاليل معملية لهم شملت سرعة الترسيب و بروتين سي المتفاعل و نسبة مضاد إنزيم ستربتوليسين و تم عمل نوعية جين تناسق أنسجة خلايا الدم البيضاء.
و قد أسفرت النتائج عن أن المرضى المصابين بالحمى الروماتيزمية يوجد بينهم و بين المجموعة الضابطة فارق إحصائي واضح في جين تناسق خلايا الدم البيضاء (أ 2)، (بي دبليو 4)، (بي دبليو 6) و في الحالات المصابة بالتهاب المفاصل قد وجد فارق إحصائي في جين تناسق أنسجة خلايا الدم البيضاء (أ 23)، (بي دبليو 4)، (بي دبليو 6)، (بي 14).
و بهذا وجدنا ارتباطا بين نوع جين تناسق أنسجة خلايا الدم البيضاء و الإصابة بالحمى الروماتيزمية.
و يوصى بعمل دراسات على عينات أكبر من المرضى لتحديد نوعية جين تناسق أنسجة خلايا الدم البيضاء المسئول عن مرض الحمى الروماتيزمية في منطقتنا و ذلك سوف يساعد مستقبليا في منع الإصابة بهذا المرض في الأشخاص المعرضين جينيا للإصابة به.
Abstract EN
Rheumatic fever is an immunological reaction to group A-β hemolytic streptococci in individuals who have a genetic predisposition.
Aim of work : To determine the HLA-antigens responsible for the occurrence of rheumatic fever, with a view to early prevention of the disease in genetically susceptible individuals in the future.
Methodology : Twenty rheumatic fever patients and 20 matched controls were studied and HLA typing was performed for all of them using the microlymphotoxicity test.
Results : There was a significantly higher HLA- A2, Bw4 and Bw6 alleles in rheumatic fever patients as compared to controls.
Patients who presented with arthritis showed significantly higher HLA- A23, Bw4, Bw6 and B14 alleles.
Conclusion : This proves the association between HLA class I antigens and the occurrence and susceptibility to rheumatic fever.
However further studies on larger scales are recommended to determine the specific alleles in our area, hence prevention of genetically susceptible individuals could be achieved in the future.
