الهدف : كان الغرض من هذا البحث معرفة خاصية و أهمية الأجسام المضادة ضد بي2- جليكوبروتين 1 و الكارديوليبين في مرضى الذئبة الحمراء الجهازية و متلازمة مضاد الفوسفوليبيد الأولية.
و فحص العلاقة الممكنة بين هذه الأجسام المضادة و بين الأعراض الإكلينيكية الرئيسية لمتلازمة مضاد الفوسفوليبيد.
الطريقة : تمت دراسة 34 مريض بالذئبة الحمراء و 10 مرضى بمتلازمة مضاد الفوسفوليبيد الأولية و تم البحث فيهم عن الأجسام المضادة للكارديوليبين (IgG) و (IgM) و كذلك بي 2 جليكوبروتين 1 (IGg) و (IgM).
النتائج : و قد وجد أن تخصص وجود الأجسام المضادة لبي 2 جليكوبروتين 1 كانت 97 % و أهميته الموجبة كانت 58 % و بين المرضى التي تحتوي دمائهم على الأجسام المضادة للكارديوليبين خاصيته 90 % و أهميته 80 %.
كما كان وجود الأجسام المضادة للكارديوليبين ذو دلالة إحصائية (0.05 > p) بتجلط الدم و ذو دلالة إحصائية عالية لتكرار فقد الجنين (0.01 > p).
أما الأجسام المضادة لبي 2 جليكوبروتين 1 كان ذو دلالة إحصائية عالية مع تجلط الدم (0.01 > p) و دلالة إحصائية مع تكرار فقد الجنين (0.05 > p) الاستنتاج : نستخلص من هذا البحث أن الأجسام المضادة لبي 2 جليكوبروتين 1 توجد بخاصية أعلى من الأجسام المضادة للكارديوليبين في مرضى الذئبة الحمراء و متلازمة مضاد الفوسفوليبيد الأولية.
و أنها تكون ذو قيمة عالية مع الأعراض الإكلينيكية في حالة وجود الأجسام المضادة للكارديوليبين أي أن وجودهم معا يؤثر في التجلط الدموي و تكرار فقد الجنين عند المرضى.
و هذا يرجع وجود أكثر من عامل يؤثر في الأعراض الإكلينيكية و شدتها لهذا المرض (مضاد الفوسفوليبيد الأولي أو تابع للذئبة الحمراء).
Abstract EN
Objectives: To define the specificity and value of anti- ?2 -glycoprotein1 (anti-?2-GP1) antibodies and anticardiolipin (ACL) in Systemic Lupus Erythematosus (SLE) and Primary Antiphospholipid Syndrome (PAPS) patients.
Also, to examine the possible association between anti-?2-glycoprotein1 (anti-?2-GP1) antibodies and anticardiolipin (ACL) and the main clinical manifestations of the Antiphospholipid Syndrome (APS).
Methodology: We studied 34 SLE and 10 (PAPS) patients.
We determined the presence of (ACL) and (anti-?2-GP1) immunoglobulin IgG and IgM.
ACL IgG and IgM were detected separately using commercial ELISA kits.
Anti-?2-GP1 antibodies were detected with ELISA using ?2-GP1.
Patients’ clinical data were reviewed separately.
Results: The specificity for anti-?2-GP1 in APS was 97%, with positive predictive value (PPV) of 58%.
Among the ACL-positive patients, specificity was 90% and PPV 80%.
ACL was significantly associated with venous thrombosis (p<0.05) and fetal loss (p<0.01).
A ?2-GP1 was associated with venous thrombosis (p<0.01) and fetal loss (p<0.05).
Conclusions: This study showed that anti-?2-GP1 antibodies have a higher specificity than ACL in SLE and PAPS patients.
The positive predictive value (PPV) of anti-?2-GP1 was greater in ACL-positive patients than in ACL-negative patients.
The association of anti-?2-GP1 and ACL (IgG and IgM) antibodies with the different clinical manifestations (i.e.
thrombosis and fetal loss) may suggest that more than one mechanism may be involved in the pathogenesis of thrombosis and fetal loss.
