الهدف : أجريت هذه الدراسة لاستكشاف وجود علاقة ممكنة بين الرتروفيروس الآدمي الخامس و مرض الرثيان المفصلي و كذلك علاقته مع كل من درجة نشاط المرض و شدته في محاولة لاستكشاف دوره المحتمل كعامل مسبب للمرض.
الطريقة : أجريت هذه الدراسة على 31 مريض بمرض الرثيان المفصلي و مجموعة ضابطة من الأصحاء تتكون من 15 فردا و لقد تم أخذ التاريخ المرضي الكامل للمرضى و عمل الفحص، الإكلينيكي الدقيق و كذا الفحوص المعملية و الأشعات التشخيصية اللازمة.
كما تم عمل تفاعل سلسلة البولميز لاستكشاف وجود الرتروفيروس الآدمي الخامس أو غيابه لجميع المرضى و المجموعة الضابطة.
و كذلك تسع عينات للسائل الزلالي من مرضى الرثيان المفصلي.
النتائج : وجود الرتروفيروس الآدمي الخامس في دم مرضى الرثيان المفصلي بنسبة هامة إحصائيا.
وجود الرتروفيروس الآدمي الخامس في أربع عينات للسائل الزلالي من التسع عينات.
المرضى الذين لديهم الرتروفيروس الآدمي الخامس في السائل الزلالي إيجابي وجد أيضا عندهم الرتروفيروس الآدمي الخامس إيجابي في الدم.
ارتفاع كل من إنزيم الألكين فوسفاتيز و سرعة الترسيب و عدد كرات الدم البيضاء بنسبة هامة إحصائية في حالات الرتروفيروس الآدمي الخامس الايجابي في الدم و السائل الزلالي.
و قد تبين من هذا إصابة ما في الكبد و التي قد تكون بسبب الفيروس نفسه.
لم يوجد فرق هام إحصائي بين درجة نشاط المرض و شدته في الدم المحتوي على الرتروفيروس الآدمي الخامس الايجابي و كذلك السائل الزلالي مقارنة بالحالات السلبية.
الاستنتاج : إن وجود الرتروفيروس الآدمي الخامس بنسبة هامة إحصائيا في مرضى الرثيان المفصلي في هذه الدراسة (25.8 %) لابد أن تؤخذ في الاعتبار حيث أنه قد يكون له دور كعامل مسبب لمرض الرثيان المفصلي.
Abstract EN
Objective : this study was carried out to detect the possible association of human retroviruse-5 (HRV-5) with RA and its correlation with disease activity and severity in a trial to detect its possible role as an etiologic factor.
Methodology : We conducted our study on 31 RA patients and 15 apparently healthy subjects serving as a control group.
All patients were subjected to complete history taking, thorough clinical examination, radiological and laboratory tests.
Nested PCR was done to detect the presence or absence of HRV-5 in the blood of all patients and controls as well as nine synovial fluid samples from RA patients.
Results : there was a significant presence of HRV-5 in the blood of RA patients and in four synovial fluid samples out of nine.
The four patients whose synovial fluid samples were HRV-5 positive also showed HRV-5 positive blood.
There was a statistically significant higher alkaline phosphatase mean in cases with HRV-5 positive blood, which may reveal Liver damage that might be caused by the virus itself.
There was no statistically significant difference in the activity or severity grades in HRV-5 positive blood and synovial fluid compared to HRV-5 negative cases.
Conclusion : the presence of HRV-5 in a significant percent in our RA patients (25.8 %) should be taken into consideration since it may have a role as an etiologic factor in the pathogenesis of this disease.
