يلعب الجهاز الدهليزي دورا رئيسيا في الاحتفاظ بالاتزان.
و قد حظي التأهيل كوسيلة مساعدة في علاج الدوار بالاهتمام منذ فترة غير قصيرة و لكن لم يتم إدخاله كوسيلة علاجية إلا في الآونة الأخيرة حيث، أثبتت الدراسات الحديثة بواسطة استعمال أجهزة التسجيل الوضعي الديناميكي المبرمج دوره الفعال في علاج المرضى الذين يعانون من الدوار و صعوبة الاتزان.
و قد تم إجراء هذا البحث علي تسعة عشر مريضا بالدوار نتيجة لاضطراب في الجهاز الطرفي الدهليزي تم تشخيصهم بواسطة جهاز الاتزان (ENOG) و قد تم إجراء برنامج التأهيل لخمسة عشر مريضا فقط و هم الذين أظهرت نتائج جهاز التسجيل الوضعي الديناميكي المبرمج (CPD) اضطرابا في الاتزان حتى يمكن متابعة تأثير البرنامج التأهيلي عليهم.
و قد استهدف البحث تقييم جهاز التسجيل الوضعي الديناميكي المبرمج كوسيلة لتحديد نوع و شدة الاضطراب بالاتزان و مدى صلاحية برامج التأهيل في علاج مرضى اضطراب العصب الطرفي الدهليزي.
و قد تم عمل استبيان للرأي لجميع المرضى لتقدير شدة الدوار و تأثيره علي ممارستهم لأنشطة الحياة اليومية كما قد تم قياس اتزانهم بواسطة جهاز التسجيل الوضعي الديناميكي المبرمج قبل و بعد برنامج التأهيل و الذي استمر لمدة ستة أسابيع.
و شمل هذا البرنامج تمرينات لتحسين الثبات الوضعي و الثبات التحديقي و أيضا تمرينات لزيادة اللياقة البدنية مع التركيز لكل مريض على التدريبات التي يلاقي فيها نوعا من الصعوبة.
و أثبتت نتائج البحث وجود فرق ذي دلالة إحصائية بين مجموعة المرضى و الأصحاء (و عددهم خمسة عشرة) في الوضعين رقمي 5، 6 و أيضا في التسجيل المركب قبل العلاج.
كما أظهرت النتائج تحسنا ذي دلالة إحصائية في قياسات الوضعين رقمي 5، 6 و التسجيل المركب بعد إجراء برنامج التأهيل و كذلك في استبيان الرأي.
و هذه النتائج تتفق مع الدراسات العلمية مثل هوراك (1991)، شبرد (1993) و غيرهم.
و توصي الدراسة باستخدام جهاز التسجيل الوضعي الديناميكي المبرمج في تقييم حالات الدوار المختلفة و باستخدام التأهيل كوسيلة فعالة في علاج حالات اختلال التوازن الدهليزي الطرفي كما يمكن أيضا استخدامه لعلاج الاتزان نتيجة الاضطرابات الجهاز العصبي المركزي لما له من نتائج مشجعة.
Abstract EN
Objective: Vestibular rehabilitation although known for long years, has gained recent interest after the development of computerized dynamic posturagraphy as an objective method of evaluation of vestibular function.
The aim of this work was to evaluate the efficacy of vestibular rehabilitation program in the treatment of peripheral vestibular disorders.
Patients & Methods: Nineteen patients and 15 normal controls were included in the study.
All patients complained of the sense of vertigo or imbalance and unilateral caloric weakness with or without directional preponderance diagnosed with electro-nystagmography (ENOG).
Compound dynamic posturographgy (CDP) was done to all cases with positive ENOG where only 15 cases showed abnormal CDP.
Those received daily one-hour customized vestibular rehabilitation program and were followed up clinically and with CDP after six weeks.
Results: Patients showed abnormalities in the sensory organization test of CDP in conditions 5& 6 and also in the composite score.
CDP follow up revealed a statistically significant improvement in these parameters following the customized rehabilitation program.
Conclusion: Vestibular rehabilitation is a useful line of treatment in peripheral vestibular disorders.
