Primitive neuroectodermal tumors are part of the Ewings’ Sarcoma family of tumors which includes highly malignant tumors sharing the feature of round small cell in histopathology.

Theses tumors involve the skeletal and extraskeletal tissue, most importantly is chest wall involvement.

We present a case of a young adult who was diagnosed with intrapleural masses, which proved to be of primitive neuroectodermal origin.

Histopathology was crucial in the diagnosis, and elimination of differentials.

On the other hand, PET-CT imaging played an important role in primary staging, diagnosis and proper assessment of tumor extension level.
