الافتراضات : ظهر في الآونة الأخيرة العديد من الأبحاث التي تتحدث عن إصابة الكلى في مرضى التهاب الفقار القسطى الحسأة.
و يمكن أن تنشأ إصابة الكلى عن أسباب متعددة منها : أ- استخدام العقاقير الغير كورتوزنية في علاج هذا المرض.
ب- إصابة كبيبات الكلى خاصة بسبب ترسب الأجسام المضادة من نوع "أ" في صورة مركب مناعي.
ت- أو نتيجة ترسب نظير النشا "داء القشوانية".
الطريقة : في هذا البحث تم تقييم وظائف الكلى في 30 مريض من مرضى التهاب الفقار القسطى للوقوف على حجم هذه المشكلة في المرضى المصريين، و كانت نتائج البحث كالآتي :- 1- أظهرت وظائف الكلى أن خمسة مرضى فقط كانت لديهم علامة أو أكثر من علامات إصابة الكلى بنسبة 17 % و من مجموع المرضى.
2- كل هؤلاء الخمسة مرضى كان لديهم دم في البول لدى الفحص الميكروسكوبي للبول( > 3 خلايا / القوى العظمى).
3- وجد أن ثلاثة مرضى ارتفعت لديهم نسبة الكرياتنين في الدم > 125 ملى مول / لتر.
4- أربعة مرضى ظهر عندهم انخفاض في معدل إزاحة الكرياتنين < 80 مم / دقيقة.
5- أربعة مرضى كان لديهم زيادة في أخراج الألبيومين في البول > 3 جم / 24 ساعة.
6- ن أثيتيل جلوكوزاميدز إنزيم ليسوسومى لأنابيب الكلى وجد مرتفع في ثلاثة مرضى من هؤلاء الخمسة.
7- البروتين مرتبط الرتينال اختبار حساس لاختلال وظيفة الأنابيب القريبة للكلى لم يكن هناك تغيير في مستواه في هؤلاء المرضى.
الخلاصة : إن وجود اختلال في وظيفة الكلى في 17 % و من مرضى التهاب الفقار القسطى يعتبر مشكلة جديرة بالاهتمام و البحث و عليه يجب متابعة وظائف الكلى في مرضى التهاب الفقار القسطى.
Abstract EN
Hypothesis: There have been many reports suggesting renal abnormalities in ankylosing spondylitis (AS) patients.
Possible mechanisms include the effect of non-steroidal anti-inflammatory drugs (NSAIDs), an increased incidence of glomerulonephritis particularly that associated with the deposition of IgA containing immune complexes and the deposition of amyloid.
Methodology: We assessed renal function in 30 AS patients randomly selected from those attending the Rheumatology Clinic of Zagazig University Hospitals, using sensitive tests of glomerular and tubular function.
Results: Five of the thirty patients presented one or more signs of renal involvement.
Microscopic hematuria in 5 patients, decreased renal function assessed by serum creatinine in 4 patients, disturbed creatinine clearance in 3 patients, microalbuminuria in 4 patients and N-acetyl-D-glucosaminidase (NAG) in 3 patients.
None of the patients presented increased urinary excretion of retinal binding protein (RBP).
Conclusion: The finding of renal involvement in 17 % of our patients suggest that renal abnormalities in AS patients are surprisingly common and should be investigated
