أجريت هذه الدراسة على 40 مريض بالتهاب المفاصل التفاعلي و 20 شخص طبيعي، و تم فحصهم إكلينيكيا و معمليا لمعرفة دور المولد المضاد للبكتريا في طبيعة مرض التهاب المفاصل التفاعلي.
تم أخذ عينات من الدم لفحصها لوجود عامل الرثيان و نوع النسيج، كما تم فحص عينات من البراز و مسحة بولية للمرضى لإجراء تحليل بي-سي-آر عليهم ضد كلامديا دي-أن-إيه.
و لقد أثبتت النتائج وجود الميكوبلازما في المسحة البولية بنسبة 20 % و وجود تحليل بي-سي-أر عليهم ضد كلامديا دي-إن-إيه بنسبة 47.5 %.
كما أثبت أيضا سبق الإصابة في الجهاز الهضمي بالبكتيريا كمبيلو بأكثر بنسبة 25 % و وجود شيذللة بنسبة 12.5 % و السلمونلة بنسبة 25 %.
ويستخلص من هذا البحث وجود دور للمولد المضاد للبكتيريا في مرضى التهاب المفاصل التفاعلي و خاصة في حالة إصابة الجهاز الهضمي بالسلمونيلا أو إصابة الجهاز البولي التناسلي بالكلاميديا و لكن يحتاج لتأكيد هذا الدور أبحاث مستقبلية يتم فيها أخذ عينة من الغشاء السينوفي و عمل تحليل بي-سي-آر لأنواع عديدة من البكتيريا التي يعتقد وجود دور لها في حدوث هذا المرض.
و إذا تأكد هذا الدور سوف يكون هناك ضرورة لاستخدام مضادات الميكروب المسبب كعلاج أساسي لالتهاب المفاصل التفاعلي.
Abstract EN
Materials: The study was performed on 40 reactive arthritis patients and 20 healthy controls.
Clinical and laboratory evaluation was used to elaborate the bacterial antigens role in the pathogenesis of reactive, arthritis.
Methods: Serum samples were collected for rheumatoid factor, tissue typing and Widal test.
Stool samples were collected for culture.
Urogenital swabs were taken for culture and polymerase chain reaction (PCR) for chlamydia-DNA.
Results: There was preceding infection of the urogenital tract through positive urogenital swab cultures of mycoplasma in 8 patients (20%), positive PCR for chlamydia-DNA in 19 patients (47.5%) and previous gastro-intestinal infection through positive compylobacter culture in 10 patients (25%), shigella in 5 patients (12.5%) and salmonella in 10 patients (25%).
There was some patients had combined infection with different types of organism.
Conclusion: There was a role of bacterial antigens in the pathogenesis of reactive arthritis, which affect gastero-intestinal tract such as salmonella and urogenital tract such as chlamydia trachomatis.
