قمنا بعمل دراسة مستقبلية عن علاج كسور أسفل عظمة الكعبرة الغير ثابتة و المتصلة بالمفصل بواسطة التثبيت بكل من جهاز التثبيت الخارجي والأسلاك المعدنية مع بداية العلاج الطبيعي تحت الإشراف الطبي منذ اليوم الأول بعد العملية.
شمل البحث عدد 42 كسرا بينهم 17 امرأة و 25 رجلا، و كان متوسط أعمارهم 47.3 سنة قمنا بتثبيت جهاز التثبيت الخارجي بين عظمة الكعبرة و العظمة المشطية الثانية لليد و قوينا التثبيت بأسلاك معدنية، رفعنا جهاز التثبيت الخارجي عند أربعة أسابيع بعد العملية و رفعنا و الأسلاك المعدنية عند متوسط 7.1 أسبوع بعد العملية.
أظهرت النتائج أن 39 مريض (92.8 %) كانت ممتازة و جيدة من الناحية التشريحية بينما كان هناك التئام معيب في ثلاثة كسور.
حصل جميع المرضى على نتائج وظيفية مرضية و عند المتابعة النهائية (بمتوسط 28 أسبوع) لم تكن هناك أي شكوى من ألم بالرسغ و كان متوسط قوة القبضة 96 % من الطبيعي.
استنتجنا من البحث أن علاج كسور أسفل عظمة الكعبرة الغير ثابتة و المتصلة بالمفصل بواسطة التثبيت بكل من جهاز التثبيت الخارجي و الأسلاك المعدنية مع بداية العلاج الطبيعي تحت الإشراف الطبي منذ المراحل الأولى للعلاج هو طريقة فعالة و باستطاعتها أن تعطي نتائج أفضل من استخدام جهاز التثبيت الخارجي بمفرده.
Abstract EN
Materials & Methods: Forty-two unstable intra-articular distal radius fractures were treated with primary external fixator augmented with Kirschner wires fixation.
An external fixator was inserted between the radius and the second metacarpal, and ligamentotaxis was maintained for 4 weeks.
A supervised physiotherapy regimen was started from day one postoperative.
The external fixation was removed at 4 weeks and Kirschner wires were removed at an average of 7.1 weeks.
Results: Seventy patients were females and 25 were males, with an average age of 47.3 years.
In 39 patients (92.9%) the anatomical result was good or excellent, but three fractures healed with malunion.
All patients made a satisfactory functional recovery, at a mean follow-up of 28 months after injury.
None had pain in the wrist and all were satisfied with the result.
The average grip strength was 96% of normal.
Conclusion: External fixation supplemented with Kirschner wires fixation with early removal of the external fixator and mobilization is an effective method for the treatment of unstable intra-articular distal radius fractures.
It could give better functional results than the use of an external fixator alone.
