يهدف هذا البحث إلى قياس مستوى الجزيء اللاصق بين الخلايا-1 والجزيء اللاصق الخلوي الوعائي -1 الذائب في مصل الرثيان المفصلي و دراسة علاقتهما بنشاط المرض.
اشتمل البحث على 40 مريضا بالرثيان المفصلي و 20 شخصا من الأصحاء كمجموعة ضابطة للمقارنة و قد أجري لهم فحص إكلينيكي شامل و الاختبارات المعملية الآتية : 1.
صورة دم كاملة.
2.
سرعة ترسيب كريات الدم الحمراء.
3.
مستوى بروتين (سي) التفاعلي.
4.
المعامل الرثياني في المصل.
5.
مستوى الجزيء اللاصق بين الخلايا-1 و الجزيء اللاصق الخلوي الوعائي-1 الذائب في المصل بطريقة إليزا.
و قد أظهرت النتائج وجود زيادة ذات دلالة إحصائية في مستوى كل من الجزيء اللاصق بين الخلايا-1 و الجزيء اللاصق الخلوي الوعائي-1 في مصل مرضى الرثيان المفصلي عنهما في المجموعة الضابطة، و قد وجدت أيضا علاقة ذات قيمة إحصائية بين مستوى الجزيء اللاصق بين الخلايا-1 و دلائل نشاط المرض مثل معامل ريتشي لقياس التهاب المفاصل و سرعة ترسيب كريات الدم الحمراء و مستوى بروتين (سي) التفاعلي، بينما لا توجد علاقة ذات دلالة إحصائية بين مستوى الجزيء اللاصق الخلوي الوعائي-1 و دلائل نشاط المرض.
نستنتج من ذلك أن مستوى الجزيء اللاصق بين الخلايا-1 يعتبر دليلا مفيدا في تقييم نشاط مرض الرثيان المفصلي و لكنه يحتاج الى تقييم بعناية في دراسات طولية.
Abstract EN
Objective: To measure serum levels of sICAM-1 and sVCAM-1 in rheumatoid arthritis patients (RA) and to determine their correlation with disease activity.
Methods: Forty RA patients and 20 healthy controls were included in this study.
They were subjected to thorough clinical examination and the following laboratory investigations: full blood picture, ESR, CRP, rheumatoid factor and serum levels of sICAM-1 and sVCAM-1 with enzyme linked immunosorbent assay (ELISA).
Results: Serum levels of sICAM-1 and sVCAM-1 were significantly raised in RA patients compared with healthy controls (p<0.001).
Serum levels of sICAM-1 showed significant correlations with parameters of disease activity including Ritchie articular index, ESR and CRP while sVCAM-1 levels did not correlate with parameters of disease activity.
Conclusion: Serum levels of sICAM-1 may be considered a useful marker of disease activity in RA and will require careful evaluation in longitudinal studies.
