يهدف هذا البحث إلى قياس مستوى النيوبترين في بول مرضى الذئبة الحمراء و دراسة علاقته بنشاط المرض.
اشتمل البحث على 40 مريضا يعانون من مرض الذئبة الحمراء و 20 شخصا من الأصحاء كمجموعة ضابطة للمقارنة و قد أجرى لهم فحص إكلينيكي شامل و الاختبارات المعملية الآتية :- • صورة دم كاملة.
• معدل سرعة ترسيب كريات الدم الحمراء.
• المعامل الرثياني في المصل.
• الأجسام المضادة للنواة.
• الأجسام المضادة للحمض النووي ثنائي النوعية.
• مستو ى المكمل رقم 3 و رقم 4 في المصل.
• مستوى النيوبترين في البول.
• مستو ى البروتين في البول في 24 ساعة.
و قد أظهرت النتائج وجود زيادة ذات دلالة إحصائية في مستوى النيوبترين في بول مرضى الذئبة الحمراء أثناء نشاط المرض و أثناء سكونه عنه في المجموعة الضابطة، و كان مستوى النيوبترين في بول مرضى الذئبة الحمراء أثناء نشاط المرض أعلى من مستواه في مرضى الذئبة الحمراء أثناء نشاط المرض أعلى من مستواه في مرضى الذئبة الحمراء أثناء سكونه.
و قد وجدت أيضا علاقة ذات قيمة إحصائية بين مستو ى النيوبترين في البول و دلائل نشاط المرض مثل معدل سرعة ترسيب كريات الدم الحمراء و نسبة الهيموجلوبين و مستوى المكمل رقم 3 و رقم 4 في المصل.
نستنتج من ذلك أن مستوى النيوبترين في البول يعتبر مفيدا في تقييم نشاط الذئبة الحمراء.
Abstract EN
Objectives: To measure urinary neopterin levels in systemic lupus erythematosus (SLE) patients and to determine their correlations with different parameters of disease activity.
Methods: Forty SLE patients and 20 healthy controls were included in this study.
They were subjected to thorough clinical examination and the following laboratory investigations: full blood picture, ESR, 24 hours urinary protein, rheumatoid factor, antinuclear antibodies (ANA), anti-double stranded DNA antibodies (anti-dsDNA antibodies) using indirect immunofluorescence, serum complement (C3 and C4) using single radial immunodiffusion and urinary neopterin using radioimmunoassay (RIA) method.
Results: Urinary neopterin levels were significantly raised in patients with active and inactive SLE as compared to healthy controls (p<0.001 and p<0.01 respectively).
It was significantly higher in patients with active than in those with inactive SLE (p<0.01).
Urinary neopterin levels showed significant correlations with parameters of disease activity including ESR, hemoglobin %, serum C3 and C4.
Conclusions: Urinary neopterin levels could be considered a useful marker for monitoring disease activity in SLE patients.
