الهدف من البحث : أجرى هذا البحث لدراسة مستوى عامل النمو التحويلي ب " 1 " و انترليوكين " 6 " في مصل الدم في مرضى الالتهاب الفقاري التيبسي و تقييم علاقتهما بالظواهر الإكلينيكية و درجة نشاط المرض.
مواد و طرق البحث : أجري هذا البحث على 19 مريضا مصابون بمرض الالتهاب الفقاري التيبسي و كانوا 15 رجل و 4 نساء تتراوح أعمارهم بين 20-40 عاما بمتوسط 29 عاما (مجموعة المرضى) بالإضافة إلى 7 من الأفراد الأصحاء تم اختيارهم في معدل العمر و النوع المناسب مع المرضى (المجموعة الضابطة) و قد تم فحص جميع المرضى فحصا إكلينيكيا دقيقا لكي نتمكن من قياس معدل نشاط المرض باستخدام BASDAI.
و كذلك تم عمل الاختبارات المعملية الآتية للمجموعتين.
1- قياس مستوى عامل النمو التحويلي ب " 1 " و إنترليوكين " 6 " في مصل الدم باستخدام الـ ELISA.
2- صورة دم كاملة.
3- سرعة ترسيب الدم.
4- معدل البروتين النشط في الدم.
نتائج البحث : 1- و قد أظهرت نتائج هذه الدراسة أن مستوى كل من عامل النمو التحويلي ب " 1 " و انترليوكين " 6 " يزداد بصورة واضحة في مجموعة المرضى عن المجموعة الضابطة.
2- يختلف مستوى زيادتهما على حسب الظواهر الإكلينيكية للمرض حيث يزيد مستوى عامل النمو التحويلي ب " 1 " في المرضى الذين يعانون من آلام الظهر بينما يزداد مستوى إنترليوكين " 6 " لصورة واضحة أيضا في مرضى التهاب المفاصل الطرفي.
3- لم نجد أية علاقة ذات دلالة إحصائية بين مستوى كل منهما و بين درجة نشاط المرض كما لا توجد علاقة أيضا مع عمر هؤلاء المرضى أو مع مدة معاناتهم من المرض.
بينما توجد علاقة ذات دلالة إحصائية بين مستوى كل منهما و بين سرعة ترسيب الدم و معدل البروتين القشط في الدم.
الاستنتاج : يعتبر قياس مستوى عامل النمو التحويلي ب " 1 " في مرضى الالتهاب الفقاري التيبسي الذين يعانون من آلام الظهر له أهمية كبرى و التحكم في مستوى هذا العامل من الممكن أن يؤثر على مجرى و طبيعة المرض و كذلك على تيبس مفاصل الظهر.
و كذلك قياس مستوى إنترليوكين " 6 " يعتبر من أكثر الدلالات حساسية لهذا المرض أكثر من قياس سرعة ترسيب الدم أو معدل البروتين النشط في الدم.
Abstract EN
Both TGF- and IL-6 play a critical role in Ankylosing Spondylitis (AS).
The present work aimed at assessing the plasma level of each and evaluating its relationship with clinical features, disease activity and routine acute phase reactants (ESR, CRP).
The study comprised 19 patients suffering from AS.
They consisted of 15 males and 4 females.
Their mean age was 29 years.
Seven healthy individuals age and sex matched were included as a control group.
All patients were clinically examined to obtain the “Bath ankylosing spondylitis disease activity index (BASDAI)”.
The following investigations were performed for both groups: TGF-1 and IL-6 by ELISA complete blood picture, ESR and CRP titre.
Both TGF- and IL-6 were found to be elevated in the AS group than the control group (p < 0.001), TGF-1 more so in patients with back pain but IL-6 more in patients with peripheral arthritis.
No correlation was found between any of them and disease activity, age or disease duration.
But both of them showed a significant correlation with ESR and CRP.
IL-6 was found to be elevated in the AS group more than ESR or CRP.
