تعتبر الأجسام المضادة لسيتوبلازم الخلايا المتعادلة نوعا جديدا من الأجسام المضادة لخلايا الدم، و قد تم اكتشاف هذه الأجسام المضادة في الأعوام الأخيرة في عديد من أمراض اضطراب الجهاز المناعي على وجه العموم و مرض الرثيان المفصلي على وجه الخصوص.
أجريت هذه الدراسة لتحديد ما إذا كانت هذه الأجسام المضادة ذات درجة عالية من الحساسية في الكشف عن مرض الرثيان المفصلي و كذلك ما إذا كان لها علاقة بالأعراض الإكلينيكية لهذا المرض خاصة التهاب الأوعية الدموية، و قد أجريت هذه الدراسة على عشرين مريضا من مرضى الرثيان المفصلي بالإضافة إلى عشرة أشخاص أصحاء كمجموعة مقارنة.
تم اختبار وجود هذه الأجسام المضادة باستخدام طريقة الفلوريسين غير المباشر، و قد أسفرت النتائج عن وجود هذه الأجسام المضادة في 25 % من المرضى و عدم وجودها بالمرة في المجموعة المقارنة.
و كانت هذه الأجسام المضادة كلها من مجموعة الجانب خلوية فقط (بي-أنثا) و غياب مجموعة السيتوبلازم الخلوية (سي-أنثا) في جميع المرضى، كما أثبتت النتائج أيضا عدم وجود أي فارق ذو دلالة إحصائية في الأعراض الإكلينيكية و التحاليل المعملية للمرض من حيث تواجد هذه الأجسام المضادة أو عدم تواجدها إلا انه فقط وجد فارق ذو دلالة إحصائية في زمن المرض حيث أنه وجد أن نسبة هذه الأجسام المضادة مرتفعة أكثر عند المرضى المزمنين (ذو مدة طويلة بالمرض).
و يمكن أن نستخلص من هذه الدراسة أن وجود هذه الأجسام المضادة في مرضي الرثيان المفصلي ليس غير عادي مع أغلبية وجود مجموعة (بي أنثا)، و أن هذه الأجسام المضادة لها علاقة فقط بأن المرض يعتبر عملية التهاب مزمن أكثر من علاقتها بنشاط المرض أو وجود التهاب في الأوعية الدموية في مرضى الرثيان المفصلي.
Abstract EN
Antineutrophil cytoplasmic antibodies (ANCA) constitute a new class of autoantibodies against leukocytes.
In recent years, they have been detected in a variety of autoimmune disorders in general and in rheumatoid arthritis in particular.
The present study was carried out to detect the sensitivity and specificity of ANCA in patients with RA and whether they have any clinical correlation, in particular the presence of vasculitis.
20 patients with RA in addition to 10 normal individuals, who served as controls, were included in this study.
ANCA is related to the chronic inflammatory process rather than a specific marker for vasculitis or disease activity in RA.
ANCA were tested by indirect immuno-fluorescence assay (IIF).
They were detected in 5 patients with RA (25%) and none of the control group.
Sensitivity was 25% and specificity was 100%.
All ANCA +ve cases showed a perinuclear pattern of staining (P-ANCA), while the diffuse cytoplasmic pattern (C-ANCA) was not detected in any of the studied subjects.
There was no significant difference (p>0.05) between clinical and laboratory findings, including the presence of vasculitis and disease activity in ANCA +ve and -ve patients except for disease duration, where ANCA +vity was significantly higher with longer disease duration (p<0.05).
We concluded that ANCA was not an uncommon finding in patients suffering of RA with predominance of a perinuclear pattern (P-ANCA).
