Background.
Cholesteatoma surgery aims to prevent cholesteatoma recidivism (recurrent and residual disease), to stabilize or improve hearing and to achieve a dry ear free from otorrhea .
Purpose.
To evaluate the value of using partial obliteration with canal wall down technique to minimize the incidence of cavity problems in cholesteatoma surgery.
Study Design.
Prospective case series.
Methods.
A modified one stage technique for cholesteatoma surgery was used.
It included retrograde atticoantrotomy extending to mastoidectomy according to cholesteatoma extent, using endoscope to detect cholesteatoma in hidden areas and hence its removal, cartilage grafting with ossicular reconstruction, then partial cavity obliteration.
Second look surgery was done for patients with suspected recidivism.
Results.
Eighty patients (80 ears) were treated from middle ear cholesteatoma.
The age ranged from 6 to 55 years, 44 were males and 36 were females.
We classified the mastoid cavity after obliteration into four grades (from 0 to 3).
fifty eight (72.5%) cases were noted as grade 0 ; ten (12.5%) cases as rade 1; seven (8.75%)cases as grade 2 and five (6.25%)cases as grade 3.
Second look suregry was needed for 10 patients (12.5%), and cholesteatoma recidivism was found only in 2 patients (2.5%).
Conclusion.
Using partial obliteration with canal wall down mastoidectomy for cholesteatoma helped to achieve a dry ear and markedly decreased the cavity problems.
