This study aimed at assessing the level of soluble intercellular adhesion molecule-1 (sICAM-1) in patients with juvenile rheumatoid arthritis (J.R.A) and to study the relationship between clinical disease activity and sICAM-1 level.
30 patients with J.R.A.
(7 males and 23 females), mean age was (16.45years) and 14 healthy asymptomatic controls, age and sex matched, were included in this study, patients were examined clinically to evaluate their disease activity.
The following investigations were done for both groups included in the study: measurement of serum levels of sICAM-1, RF.
IgG and RF.
IgM by enzyme linked immunosorbent assay (ELISA), as well as Hb.
concentration, E.S.R.
first hour, C.R.P.
and rheumatoid factor by agglutination.
There was an increase in sICAM-1 level in JRA patients with a highly significant increase when compared to normal controls (P <0.001).
Only 6 out of 30 patients (30%) showed positive rheumatoid factor using Rheumatoid factor IgM and IgG were significantly higher in J.R.A.
group relative to the control (P<0.05) (14/30) of the cases showed higher IgM and IgG values respectively.
No significant correlation's were found between sICAM-1 when compared to duration of illness, criteria of disease activity (joint score, pain score, function performance) and other laboratory findings: Hb-concentration, ESR, CRP, RFIgM and RFIgG.
We could not find firm evidence that increased levels of sICAM-l reflect an active acute phase response and disease activity in J.R.A.
